On Saturday, September 10, 2011 at approximately 4:45 p.m., the Idaho State Police investigated an injury crash on US 30 at milepost 370, in front of the Sinclair gas station in Lava Hot Springs, ID.

Casey Hlavaty, 27, of Pocatello, ID, driving a white and gray 1999 Subaru Legacy Outback station wagon, was attempting to make a left hand turn into the Sunnyside Sinclair Gas Station from the eastbound lane of US 30. Danielle L. Shaffer, 24, of Inkom, Idaho was traveling behind Hlavaty in a red 1999 Honda Civic. Duane A. Quinn, 49, of Armstrong, IA, was traveling westbound in a blue Ford pickup pulling a horse trailer carrying five horses. As Shaffer was attempting to pass Hlavaty on the right, she lost control of her vehicle and struck Hlavaty's vehicle. Shaffer's vehicle then traveled into the westbound lane of travel, where it was struck by Armstrong's pickup.

Also in Quinn's vehicle was Juan Carlos Ramos-Sanchez, 49, of Malad, ID. All occupants of all vehicles were wearing seatbelts. Ramos-Sanchez, Shaffer and Quinn were transported by ground ambulance to Portneuf Medical Center in Pocatello, ID.

The crash is under investigation.
