On Thursday, January 31, 2013 at approximately 11:00 a.m., Idaho State Police investigated an injury crash on State Highway 55 at milepost 68, near Gardena Road in Boise County.

Holli Porterfield, 35 of Garden Valley, ID, was driving north on State Highway 55 near milepost 68. Porterfield was driving inattentively, crossed the lane and drove off the roadway. The vehicle rolled down the embankment coming to rest on its top. Porterfield was transported by ground ambulance to St. Alphonsus in Eagle.

Porterfield was wearing a seat belt. The lanes were blocked for approximately an hour.

INCIDENT NARRATIVE:
On January 28, 2013 at 10:46 pm the Idaho State Police initiated a traffic stop on Byron K. Foster for a traffic offense near Old Main Street in Julieatta, Latah County. Foster was subsequently arrested for felony DUI, felony possession of a controlled substance with intent to deliver, felony possession of a schedule II narcotic, misdemeanor possession of a schedule II substance, possession of paraphernalia, open alcoholic container and driving without privileges. Foster was incarcerated at the Latah County Jail. $561 dollars was also seized by the Latah County Sheriffs Office.

On January 28, 2013, at approximately 5:00 p.m., Idaho State Police investigated a commercial vehicle crash on Interstate 86, at milepost 43, near American Falls, ID.

Andrew Baker, 53, of St. Anthony, ID, was driving east on Interstate 86 near milepost 43, in a 2013 Kenworth tractor, pulling an empty 2008 Beall tanker trailer. Baker steered off the road into the median. The vehicle traveled through the median, off the top of an overpass and came to a stop partially on the roadway below the interstate.

It is unknown whether Baker was wearing a seatbelt. Baker was transported by ground ambulance to Power County Hospital District in American Falls, ID. He was later transported by ground ambulance to Portneuf Medical Center in Pocatello, ID.

The eastbound fast lane is still blocked at this time, as the tow company attempts safe removal of the vehicle.

The crash investigation is continuing.

On January 28, 2013, at 10:24AM, Idaho State Police investigated a two vehicle crash on Interstate 15 at 115, just south of the Sunnyside Exit. Tamra Wyatt, 54, of Townsend, Montana, was driving northbound on Interstate 15 in her black 2007 GMC Sierra behind a semi truck. The semi slowed down with traffic and Wyatt was unable to react in time. Wyatt rear ended the semi causing her vehicle to rotate and come to rest blocking both northbound lanes. Both the driver and the passenger were wearing their seatbelts. Portions of the northbound interstate were closed while crews worked to remove the vehicles.

On January 28, 2013, at 11:09 AM, Idaho State Police investigated a one vehicle crash on Interstate 15 at exit 116. Eric L. Platt, 64, of Spokane, Washington, was driving south on Interstate 15 in his gray 2006 Ford F-250 pulling a twenty six foot travel trailer. As Platt drove under the southbound 116 overpass, by Sunnyside Road, he encountered some ice and lost control of his truck. The trailer detached and came to rest on its side on top of the guardrail. Both the driver and the passenger were wearing their seatbelts and were not injured. Portions of the interstate were blocked for two hours while crews worked to remove the vehicles.

On January 27, 2013 at approximately 8:15 AM, the Idaho State Police investigated a one-vehicle crash on US 30 at milepost 427, 4 miles southeast of Georgetown.

Gary D. Wood, 61, of Idaho Falls, ID was driving east on US 30 in a 2007 Peterbilt tractor with a utility trailer. Wood became distracted and lost control of the vehicle. He drove off the road, and the vehicle overturned.

Wood was wearing his seatbelt and was not injured in the crash.

On January 27, 2013, at approximately 5:15 p.m., Idaho State Police investigated a one vehicle crash on Interstate 15, near milepost 118, south of the Broadway exit, near the Pancheri overpass.

Ali Subah A. Alharbi, 20, of Pocatello, ID., was driving north on Interstate 15 near milepost 118, in a black 2006 Ford Mustang. Alharbi lost control of the vehicle on icy roads, and slid off the roadway. Rashed Mohammed Dekheel Alenezi, 21, of Kuwait, was a passenger in the vehicle.

Alharbi and Alenezi were both wearing seatbelts. Alenezi was transported by ground ambulance to Eastern Idaho Regional Medical Center in Idaho Falls, ID.

Idaho Transportation Deparment has opened both westbound and eastbound lanes of Interstate 84 from mileposts 95 to 137 with some lane restrictions in place westbound between mileposts 115 to 120. Westbound lanes will be restricted from mileposts 115 to 120, throughout the evening and into the night, as tow trucks continue to work on removing semi-trucks involved in the series of crashes throughout the day.

Motorists are urged to proceed with extreme caution as roadways are still slick, crews and equipment are still working along the road, and there is reduced visibility due to fog in the area.

Idaho State Police and other local law enforcement agencies are responding to several calls of slide-offs and crashes on Interstate 84 from Boise to Mountain Home. Several vehicles are blocking the eastbound lanes of traffic near milepost 60, just east of Boise. Eastbound traffic is being diverted off and turned around at the Eisenman Exit (exit 59). Westbound traffic on Interstate 84 just east of Mountain Home, near milepost 100, is backed up for over a mile due to a crash.

Winter driving conditions are in effect. The roadway is extremely icy. Motorists are advised to slow down, increase your stopping distance, and give yourself extra of time to get to your destination.

On January 23, 2013, the Idaho State Police investigated an injury crash southbound Interstate 15 at milepost 118, near Idaho Falls. Kristi Brown, age 41, of Rexburg, was traveling south in a 2003 Kia Sedona, when her vehicle began to leave the roadway on the left shoulder. Brown overcorrected, crossed both lanes of southbound traffic, and left the roadway on the right shoulder. Her vehicle rolled through the fence and came to rest on a frontage road.

Kristi Brown and her passenger Joseph Brown, age 39, of St. Anthony, were both transported by ground ambulance to Eastern Idaho Regional Medical Center in Idaho Falls. Both occupants were wearing their seatbelts.

Idaho State Police were assisted by the Idaho Falls Police Department and the Bonneville County Sheriff's Office. The crash is under investigation by the Idaho State Police.

Floyd Eastwood, age 67, of Filer, Idaho was traveling westbound on Interstate 86 in a 2005 Toyota Corolla. Margarito Rangel, age 47, of Burley, Idaho was traveling westbound on Interstate 86 in a 2013 Freightliner pulling two trailers. Eastwood struck Rangel's second trailer from behind. Lyle Harrop, age 57, of Elba, Idaho was traveling westbound in a 1995 Kenworth commercial vehicle, when he then struck Eastwood's vehicle, which was in the lane of travel.

The westbound lane of travel was blocked for approximately one hour. The crash is under investigation by the Idaho State Police.

On January 18, 2013, at approximately 3:25 p.m., a 1998 Toyota RAV 4, driven by Amber L. Lidell-Thomas, 23, of Declo, ID, was broken down in the right hand travel lane of Interstate 84, on the Snake River Bridge, just west of the Declo Exit 216. A 2005 Peterbilt struck the rear of the Toyota. (The driver information for this vehicle is pending. An update with this information will be sent in the morning.) A 2001 Oldsmobile Alero, driven by Katie Quinn, 27, of Boise, ID, was passing these vehicles in the left hand travel lane. Oleg V. Delekh, 34, of Meridian, ID, attempted to move into the left hand travel lane in a 2006 Volvo, and struck Quinn's vehicle and the 2005 Peterbilt. A 2011 Peterbilt, driven by Keith C. Forsgren, 50, of Idaho Falls, ID, then struck the rear of the Volvo driven by Delekh.


The 2006 Volvo driven by Oleg was hauling a load of onions. The side of the trailer was ripped open, causing onions to spill onto the roadway. The eastbound lanes of travel were blocked for approximately four hours and fifteen minutes during the removal of the vehicles and the onions.

All drivers were wearing seatbelts. There were no injuries in these crashes.

On January 18, 2013, at approximately 4:10 p.m., Idaho State police investigated a vehicle fire involving a three-axle tractor trailer, carrying approximately 1500 gallons of diesel fuel. Robert L. Browning, 54, of Boise, ID. was driving east on Interstate 84 at milepost 68, east of Boise, ID. Browning experienced a mechanical problem with the truck. The truck caught on fire, and Browning was able to manuever the vehicle to the left-hand shoulder of the interstate. The vehicle and fuel became fully engulfed in flames.

Boise Fire Department and the Boise Hazardous Materials Team recommended closure of the eastbound lanes of Interstate 84 at Exit 59 (Eisenman Rd.) for the safety of the public. Vehicles caught between the closure at Exit 59 and the incident at milepost 68 were given the option of using the emergency crossover at milepost 67 to return westbound.

The Interstate was completely blocked for approximately three hours. There was partial lane blockage for approximately 6.5 hours. All lanes are now completely open.

On January 18, 2013, at approximately 4:42 pm, Idaho State Police investigated a two vehicle fatality crash on State Highway 46 at mile post 102, 2 miles north of Wendell ID.

Joaquin Figueiredo, 65 years old, of Gooding ID, was traveling north on State Highway 46, in a 2003 Dodge van, when he went off the road hitting a guard rail. Joaquin then came back across the northbound lane of travel and into the southbound lane. Joaquin hit Joseph Melguin, 45 years old also of Gooding ID, who was traveling south, in a 1992 Chevy truck, head on. Joseph was ejected from his vehicle and succumbed to his injuries on scene, notifications to the family have been made. Joaquin was extricated from his vehicle and transported to North Canyon Medical Center. The crash is still under investigation.

State Highway 46 was blocked for approximately 3.5 hours; all lanes of travel are now open.

INCIDENT NARRATIVE: Adair pulled onto US12 from the Lenore Rest Area attempting to make a left turn. Adair did not yield from the stop sign to Miller who was westbound on US12. Miller, Praest and Monaghan were transported to St. Josephs Regional Medical Center for treatment. US12 was intermittently blocked for approximately 2 hours.

On January 14, 2013 at approximately 1510 hours, Idaho State Police investigated a traffic complaint on US12 near milepost 23, where it was reported that a passenger in a taxi cab began striking the driver in the face. The passenger, later identified as Micah Lyon, was subsequently arrested for Battery, Resisting and Obstructing Officers, and Indecent Exposure. Lyon was incarcerated at the Nez Perce County Jail.

On Thursday January 10, 2013, at approximately 12:17 pm, Idaho State Police investigated a two vehicle crash, eastbound Interstate 84 near milepost 120.

Jason McCain, 39, of Kent, WA, was westbound on Interstate 84 in a 2005 Ford F150. McCain was driving too fast for the weather conditions and lost control of his vehicle. McCain crossed through the median into the eastbound lanes, hitting a 1997 Chrysler Town & Country driven by Kirk Price, 70, of Ogden,UT. Price and his passenger Dorothy Price, 69, also of Ogden,UT, were transported by air ambulance to Saint Alphonsus Regional Medical Center in Boise. McCain was transported by ground ambulance to Elmore Memorial Hospital. All three people were wearing seat belts.

The left lane of Interstate 84 was temporarily blocked and has been reopened.

On January 9th, 2013 at 10:09 a.m., the Idaho State Police investigated a fatality crash on State Highway 36 near milepost 127, just east of Dayton. A Franklin County dump truck was headed eastbound on State Highway 36. There was thick fog in the area. The dump truck struck the side of a train that was already in the railroad crossing near milepost 127. Multiple train cars were derailed, and the tracks were damaged in the crash.

The driver of the truck succumbed to their injuries at the scene. Their name is not being released at this time pending notification of the family.

Both lanes of State Highway 36 are blocked at this time. The investigation is continuing.

Ilda M. Arizmendi, 33 of Wendell, ID, was traveling east on Interstate 84 in a 1998 Dodge Caravan. Arizmendi attempted to change lanes and lost control of the vehicle, went into the median, then into oncoming traffic of westbound Interstate 84, going head-on with a 2006 GMC Sierra pickup driven by Greg S. Peasnall, 50 of Hagerman, ID. Both vehicles came to rest in the fast lane of westbound traffic.

Both drivers were transported by ground ambulance to St. Lukes Magic Valley Center.

All occupants of both vehicles involved were wearing their seatbelts and weather was a factor in the crash.

Both westbound lanes were blocked for approximately two and a half hours.

The names of the deceased are Ashton E. Braden, 17 of Caldwell, and Sofia C. Moore, 6 of Caldwell. An additional passenger, Zoey J. Moore, 11 of Caldwell, was transported by air ambulance to St. Alphonsus Regional Medical Center. None of the three girls were wearing seatbelts.

Alberto Luna and his passenger, Epigmenio A. Valenzuela, 38 of Nampa, were both wearing their seatbelts.

On January 3, 2013, at approximately 4:20 p.m., the Idaho State Police investigated a fatal crash at the intersection of Old Hwy 30 and Farmway Rd, near Caldwell.

Alberto Luna was traveling westbound on Old Hwy 30 in a farm truck when he collided with a 1992 Oldsmobile Cutlass. The Oldsmobile had 3 occupants, two of which succumbed to their injuries on scene, and the third was transported by air ambulance to St. Alphonsus Medical Center in Boise.

The names are not being released at this time, and the crash is still under investigation.

On January 3, 2013, at approximately 4:20 p.m., the Idaho State Police investigated a fatal crash at the intersection of Old Hwy 30 and Farmway Rd, near Caldwell.

Alberto Luna was traveling westbound on Old Hwy 30 in a farm truck when he collided with a 1992 Oldsmobile Cutlass. The Oldsmobile had 3 occupants, two of which succumbed to their injuries on scene, and the third was transported by air ambulance to St. Alphonsus Medical Center in Boise.

The names are not being released at this time, and the crash is still under investigation.

On Thursday January 3, 2013, at approximately 9:30 am, Idaho State Police investigated a two vehicle crash on US 26 at milepost 297, near Blackfoot, Idaho.

James Miller, 52, of Herriman, Utah, was driving west on US 26 in a 2007 Dodge Ram 3500 pickup. Mark Bynum, 52, of Blackfoot, driving a 2001 Dodge Ram 1500, was attempting a left hand turn out of a residence onto US 26. Bynum did not yield to Miller's vehicle and was struck on the driver's side. Bynum was not wearing his seatbelt and was ejected from his vehicle. Bynum was transported via air ambulance to Portneuf Regional Medical Center in Pocatello. Miller was wearing a seatbelt and was not transported.

All lanes have been cleared at this time. The crash is under investigation by Idaho State Police.

INCIDENT NARRATIVE: Chandler was traveling eastbound on US12 while negotiating a left hand curve, lost control, went through the guard rail and down the embankment. The semi tractor and trailer ended upside down, partially in the river, spilling the load of lumber it was hauling. The juvenile female passenger had to be extricated from the semi tractor. Both occupants were transported by Life Flight. Investigation continuing.

On Wednesday, January 2, 2013, Idaho State Police is investigating an injury crash that occurred this morning, just before 8:00 A.M., on Eastbound Interstate 84 at mile post 43. This is between Ten Mile and Meridian Road. A piece of wood came through a windshield seriously injuring the driver. The wood could have come from the westbound side of the Interstate. We are asking the public's assistance with any information that may help us. If you witnessed this crash or saw any vehicle losing lumber on Interstate 84 between Ten Mile and the Meridian exit before 8:00 A.M., please contact Idaho State Police dispatch at 208-846-7500.

The Idaho State Police is investigating a crash that occurred westbound on Interstate 86 at milepost 62. At approximately occurred at 2:57 pm, on January 1, 2013, a Cadillac was travelling from southbound Interstate 15 to westbound Interstate 86. At the same time a 1999 Chevrolet Blazer was travelling from northbound Interstate15 to westbound Interstate 86. The Cadillac travelled into the lane occupied by the Chevrolet Blazer and collided with the Blazer. The Blazer rolled and came to rest on its roof, while the Cadillac left the scene.

The Cadillac sustained heavy damage to the driver side and the front end of the car. It is believed to be a White 2 door model with Utah license plates that start with either A68, 638 or A38. This vehicle is also believed to have been involved in a reckless driving incident on New Year's Eve that involved brandishing a weapon. The Idaho State Police is asking anyone with information about this vehicle contact them at (208) 236-6066.
