On March 31, 2019, at approximately 2:30 p.m., Idaho State Police assisted a commercial vehicle on Interstate 84 at the exit 27 on ramp, in Caldwell. All debris has been removed and the on ramp is open.

The on ramp was blocked for approximately three and a half hours.

On March 30, 2019, at 2:43 PM, Idaho State Police investigated a fatality crash on Murtaugh Rd. approximately four miles north of Murtaugh, Idaho.

Shae Mayner, 42, of Twin Falls, ID, was riding a bicycle northeast on Murtaugh Grade when he attempted to pass a tractor driven by Sergio Villegas Alfaro, 52, of Rupert, ID. Mayner fell from his bike and was struck by the tractor. Mayner was wearing a helmet and succumbed to his injuries at the scene. Next of kin has been notified.

Idaho State Police was assisted by the Twin Falls County Sheriff's Office, Jerome County Sheriff's Office, Jerome County Coroner's Office, Hazel First Segregation Fire Department and Air Saint Luke's.

On March 29th at 06:24 am, the Idaho State Police responded to a report of a fuel tanker truck that had crashed on US12 at milepost 163 near Powell, Idaho. Gary G. Claussen, 66 yoa, of Missoula, Montana, was westbound on US12 when, for an unknown reason, went off the road at milepost 163. The brown 2005 Peterbilt tractor driven by Mr.Claussen, became disconnected from the fuel trailer and overturned, trapping Mr. Claussen inside the cab. The Idaho Transportation Department arrived on scene and assisted in removing Mr. Claussen from the vehicle. One of the fuel tanks ruptured due to the crash and spilled 1200 gallons of gasoline into the drainage ditch. This ditch connects to the reservoir which connects to the Lochsa River. Once the scene was assessed, measures were taken to contain the spill and the roadway was shut down in both directions. The Department of Environmental Quality, the Environmental Protection Agency and the Nez Perce Tribe were all notified. West Central Environmental Consultants responded to the scene, completed the initial assessment and began the clean up. The Region Two HazMat Team and the Environmental Protection Agency Liaison Officer were also on scene. Mr. Claussen was transported to St. Patrick Hospital in Missoula for evaluation of minor injuries. The road was completely blocked for approximately 3.5 hours. One lane was opened and traffic was intermittent for approximately 6 hours until the road was completely open around 6:30 pm. The fuel tanker and trailer have been removed however the clean up crew will remain in the area for approximately a week. Investigation is ongoing.

On March 28, 2019 at 8:22 p.m., the Idaho State Police investigated a two vehicle, injury crash eastbound on US20 at milepost 318, near the County Line Road exit.

Drew Weathermon, 20, of Ammon, was traveling westbound on US20 in a 2001 GMC Sierra Pickup Truck pulling a trailer. The trailer detached from the GMC, went through the median and into the eastbound lanes. Jacob Amrein, 28, of Blackfoot, was traveling eastbound on US20 in 2005 GMC Yukon pulling a flatbed utility trailer and was stuck by Weathermon's trailer. Amrein's vehicle rolled onto its side.

Amrien's passengers, Brett Stocking, 54, of Blackfoot, and Danielle Beebe, 25 of Blackfoot, and a juvenile, were transported by ground ambulance to Eastern Idaho Regional Medical Center. Weathermon was not injured. Amrien and Beebe were not wearing seatbelts. The eastbound lanes were blocked for two hours. Idaho State Police was assisted by Jefferson County QRU, Idaho Falls Fire and Ambulance, Bonneville County SO and Jefferson County SO.

On March 28, 2019, at approximately 2:22 p.m., Idaho State Police investigated a two-vehicle, injury crash westbound Interstate 84 at milepost 31, west of Nampa.

Paul Servatius, 61, of Middleton, was driving westbound on I84 in a 2019 Toyota Camry. Jennifer Loutzenhiser, 42, of Caldwell, was also driving westbound on I84 in a 2014 GMC Terrain. Loutzenhiser failed to slow for traffic conditions in the area and struck the rear of the Toyota Camry. All occupants were wearing a seatbelt.

Loutzenhiser was transported by ground ambulance to Saint Luke's Nampa Medical Center. Paul's passenger, Debra Servatius, 61, of Middleton, was also transported by ground ambulance to Saint Luke's Nampa Medical Center.

The left two lanes of travel were blocked for approximately one and a half hours.

On Tuesday March 26, 2019 at approximately 7:00 P.M., The Idaho State Police responded to an injury crash northbound on US91 at milepost 84, just north of Chubbuck.
Andrea Barlow, 68, of Pocatello, was driving a 2003 Honda Civic. Julia Cruz, 85, of Heyburn was driving a 2016 Toyota Corolla. Barlow was yielding to oncoming traffic before making a left turn when Cruz struck Barlow from behind.

Barlow and Cruz were transported by ground ambulance to Portneuf Medical Center in Pocatello by ground ambulance. The southbound lane was blocked for approximately one hour while crews worked to clear the scene.

This crash is under investigation by the Idaho State Police.

On Tuesday, March 26, 2019, at approximately 1:15 p.m., Idaho State Police investigated a two-vehicle, fatality crash, eastbound Interstate 84 at milepost 36, near the Franklin Road exit in Nampa. An adult male was driving a 2018 Ford pickup eastbound on I84 when he veered towards the median shoulder and collided with a 2006 Buick Rainier that was parked on the shoulder.

The driver of the Buick, Rosita Martinez Lopez, 35, of Mexico and her passenger, Sebastian Trujillo Varona, 57, of Mexico were both transported by ground ambulance to Saint Alphonsus Medical Center in Nampa. An additional passenger from the Buick and the passenger from the Ford, Grayhl Johnson, 82, of Philomath, Oregon were transported by ground ambulance to Saint Alphonsus Regional Medical Center in Boise. The driver of the Ford pickup succumbed to his injuries at the scene. All occupants were wearing seatbelts.

The left lane of travel was blocked for approximately two hours while crews investigated. Driver of the Ford pickup's name is not being released at this time.

On Monday, March 25, 2019, at 4:45 p.m., Idaho State Police investigated a two-vehicle fatality crash near on State Highway 31 at milepost 5.4, north of Swan Valley.

Nevada J. Chavez, 24 of Victor, was driving a 1997 Subaru Legacy Wagon northbound on State Highway 31 and failed to negotiate a curve. Chavez collided head-on with a 2018 BMW X5 that was southbound, driven by Seied Z. Yasrobi, 64 of Jackson, Wyoming.

Chavez was not wearing a seatbelt and succumbed to her injuries on scene. Next of kin has been notified.

Yasrobi, and his passengers Brenda K. Younkin, 45, and a juvenile, all from Jackson, Wyoming, were transported by ground ambulance to the Eastern Idaho Regional Medical Center in Idaho Falls. All occupants were wearing seatbelts.

State Highway 31 was blocked in both directions for 5 hours.

This crash is under investigation by the Idaho State Police with the assistance from the Idaho Transportation Department, Teton County Sheriff's Office, and the Bonneville County Sheriff's Office.

On Thursday, March 21, 2019, Idaho State Police investigated a two-vehicle injury crash, on US20 at milepost 279.5, west of Idaho Falls.

James McCurry, 44, of Idaho Falls, was traveling westbound on US20 in a 2000 Ford Ranger. Kyle Foster, 30, of Mackay, was traveling eastbound on US20, along with two juvenile passengers, in a 2003 GMC Sierra. McCurry crossed over the center line then collided head-on with Foster. Both vehicles came to rest on the south side of the road.

At 9:10 a.m. on March 20, 2019, Idaho State Police investigated a two-vehicle injury crash eastbound on US26 at milepost 357.3. Harry Keyfauver, 89, of Afton,WY was driving westbound on US26 at milepost 357.3 in a Chevrolet Impala. Jorden Voss, 25, of Rexburg was pulling out of the Clark Hill Rest Area in a Ford Van. Voss pulled in front of Keyfauver, and the two vehicles collided in the eastbound lanes. Harry and his passenger, Cynthia Keyfauver, 72 and also of Afton, WY, were both transported by ground ambulance to Eastern Idaho Regional Medical Center in Idaho Falls. Voss was transported by private vehicle. The roads were blocked for approximately one hour and twenty minutes.

On March 20, 2019 at approximately 7:14 a.m. a crash occurred on US 95 at Neider Avenue in Coeur d'Alene, Idaho. A silver 2004 Chevy Suburban driven by Christopher P. Grasseschi, 33, of Coeur d'Alene, Idaho was traveling south bound on US 95 approaching Neider Avenue when he sideswiped a black 2016 Dodge 2500 pick-up driven by Paula J. Austin, 56, of Coeur d'Alene, Idaho. Grasseschi's vehicle continued through the intersection and crossed into the northbound lanes. A black 2018 Ford Edge driven by Gloria G. Baker, 48, of Rathdrum, Idaho and a black 2012 Dodge 1500 pick-up driven by Alan. B. Cannaday, 57, of Cataldo, Idaho were both traveling north bound US 95 when they were struck by Grasseschi's vehicle in the north bound lanes of US 95 at Neider Avenue. Baker and Grasseschi were transported to Kootenai Medical Center with non-life threatening injuries. The highway was partially blocked in both directions for approximately 1 hour. The investigation continues.

On Tuesday, March 19, 2019, at approximately 3:42 p.m., Idaho State Police investigated a two-vehicle, injury crash eastbound I84 at milepost 194, west of Burley.

Travis Martin, 54, of Springville, UT, was eastbound on I84 at milepost 194 in a 2017 Volvo semi pulling a single Great Dane trailer. Donald Baker, 50, of Twin Falls, was behind the Volvo in a 2004 Honda Civic. The Volvo was slowing down for traffic due to a separate crash at milepost 195. The Honda struck the Volvo from behind.

Both drivers were wearing seatbelts. Baker was transported to Cassia Regional Hospital in Burley by ground ambulance. He was then transported by air ambulance to St. Alphonsus Regional Medical Center in Boise.

The interstate was blocked for approximately an hour before the right lane was reopened. The left lane was blocked for an additional two hours. Both lanes are now open. Idaho Transportation Department and Hazelton Fire Department both assisted at the scene of the crash.

The crash is under investigation by Idaho State Police.

On March 19, 2019 at approximately 10:20 a.m., Idaho State Police located a stolen vehicle in Bonneville County.

A 2012 GMC Acadia driven by a juvenile runaway was located at the Chevron gas station off Interstate 15 and 65th South in Bonneville County. As the Trooper attempted to stop the vehicle, the driver backed up and hit the Trooper's patrol vehicle and another vehicle that was parked. The vehicle fled and was later located by Bingham County Deputies. Bingham County Deputies attempted to stop the vehicle and the vehicle fled again, resulting in a pursuit in Blackfoot.

The vehicle struck an embankment in a parking lot and rolled. The driver and two juvenile passengers were transported by ground ambulance to Bingham Memorial Hospital. Bingham County took the juveniles into custody.

Vance E. Pulsipher, 60, of Twin Falls, was traveling eastbound on US30 in a Volvo semi-truck pulling double trailers. Francisco Castillo, 65, of Jerome, was traveling westbound on US30 in a Kenworth fertilizer truck. Pulsipher failed to yield as he was attempting to turn onto East 3700 North, and struck Castillo's semi-truck.

Both drivers were wearing a seatbelt.

Both eastbound and westbound lanes of US30 were blocked for approximately 2.5 hours. The lanes have been re-opened.

On Sunday, March 17, 2019, at approximately 12:54 a.m. Idaho State Police received a call for a wrong way driver traveling westbound in the eastbound lanes of I-84 near milepost 155, the Wendell exit. The vehicle crashed near milepost 147, the Tuttle exit at approximately 1:02 a.m.

An unknown driver and his passenger, who fled the scene on foot, were in a 2008 Saturn Vue traveling westbound in the eastbound lanes on I-84. Demetri Kondratyuk, 22, of Lincoln, NE, was driving a 2004 Audi A4 eastbound on I-84. The vehicles collided head-on near milepost 147, the Tuttle exit.

Kondratyuk and his passenger Bogdan Kondratyuk, 19, of Lincoln, NE, were transported by ground ambulance to North Canyon Medical Center in Gooding. Demetri and Bogdan were wearing seatbelts.

The eastbound lanes where blocked for approximately one and a half hours.

Idaho State Police was assisted at the scene by the Gooding County Sheriff's Office, Jerome County Sheriff's Office, Wendell Fire Department, and Gooding County Ambulance.

On Saturday, March 16, 2019, at approximately 6:37 A.M., Idaho State Police investigated a two-vehicle fatality crash, on State Highway 46 at milepost 102, two miles north of Wendell.

Daniel Cruz Ortiz, 21, of Wendell, was driving southbound on SH46 in a 2009 Dodge 1500 pick-up. Jessie Otton, 26, of Twin Falls, was driving northbound on SH46 in a 2001 Chevrolet Blazer. Cruz Ortiz fell asleep, crossed the center line and collided head on with the Chevrolet Blazer. Otton succumbed to her injuries on scene. All occupants were wearing a seatbelt.

Otton's juvenile passenger was transported by air-ambulance to Saint Luke's Regional Medical Center in Boise. The juvenile passenger was wearing a seatbelt.

SH46 was completely blocked for approximately two and a half hours.

Idaho State Police was assisted by Gooding County Sheriff's Office, Wendell Fire Department, and the Idaho Transportation Department.

On March 16, 2019 at approximately 7:02 a.m. a fatal crash occurred on US12 at milepost 58, 7 miles west of Kamiah, Idaho. A gold 2006 Jeep Cherokee driven by Douglas Hurlburt,69, of Cottonwood, Idaho was traveling westbound on US12 towards Lewiston when due to an unknown reason the vehicle left the roadway. The vehicle impacted the rock wall, flipped end over end several times coming to rest on its roof. The female juvenile passenger crawled out of the vehicle under her own power. A passerby removed Mr. Hurlburt from his seatbelt and pulled him away as the vehicle caught fire. Both occupants of the vehicle were wearing their seatbelts at the time of the crash. CPR was attempted on Mr. Hurlburt but was unsuccessful. The female juvenile was transported by ambulance to Clearwater Valley Hospital where she was treated for minor injuries and released to her mother. Next of kin have been notified. The investigation is ongoing.

On Friday, March 15, 2019, at approximately 8:11 A.M, Idaho State Police investigated a three-vehicle injury crash on N 4000 E at State Highway 48, in Rigby.

Gayla Bennick, 56, of Rigby, was traveling south on N 4000 E in a 2013 Jeep Wrangler. A Deputy from Jefferson County Sheriff's Office was traveling east on State Highway 48 in a 2018 Chevrolet Tahoe. Bennick failed to yield from a stop sign and pulled out in front of the Deputy, who then struck Bennick. They then struck Ricardo Galvan Munoz, 44, of Rigby, who was stopped at a stop sign on N 4000 E in a 2006 Nissan Armada.

The Deputy was transported to the Eastern Idaho Regional Medical Center by ground ambulance. All parties were wearing a seatbelt.

Bennick was cited for Failing To Yield from a stop sign.

The travel lane at N 4000 E was blocked for approximately two hours.

On March 9th, 2019 at approximately 8:41 pm the Idaho State Police responded to a fatal crash northbound US95 at Boekel Rd. Cory Kreager, 35 years old of Priest River, ID, was travelling eastbound on Boekel Road in a black 2008 Ford F350 when he failed to stop at the stop sign. He impacted Vern Allen, 74 of Cocolalla ID, who was travelling northbound on US95 in a gold 2003 Dodge Caravan. Kreager impacted the van on the driver's side and Ryan Allen, 38 of Cocolalla, ID, was ejected from the vehicle. Ryan Allen was pronounced deceased at the scene. All the occupants of both vehicles were wearing their seatbelts. Vern Allen and his passengers, Patricia Allen, 71 of Cocolalla, and Troy Allen, 51 of Sandpoint, were transported to Kootenai Health. Alcohol is suspected to be a factor and the investigation is ongoing.

On March 8, 2019 at approximately 7:14 pm a property damage crash occurred on Interstate 90 at milepost 8, Huetter rest area. A 2016 Volvo semi-truck carrying cargo consisting of sixteen 2,000 pound missiles driven by Leiloni Dearinger, 56, of Chickasha, OK was traveling through the eastbound Port of Entry. Dearinger inadvertently drove into the hazmat containment area instead of using the Interstate on ramp. At the end of the containment area Dearinger drove over a large snow bank causing the truck to be disabled. The interstate both eastbound and westbound was closed for approximately two hours as a precaution until the load was confirmed not a hazard. Dearinger was uninjured and received a citation for inattentive driving.

On March 7th, 2019 at approximately 07:05 a.m., an injury crash occurred on State Highway 54 at milepost 10, two miles east of Athol, Idaho. A silver 2002 Honda Civic driven by Michael A. Allen, 19, of Athol, Idaho entered westbound State Highway 54 from a private driveway. A silver 2005 Honda Civic, driven by a 16-year old juvenile, was also traveling westbound on State Highway 54 and swerved to avoid hitting Mr. Allen's vehicle. The silver 2005 Honda Civic with the juvenile driver collided, head-on, with a white 2006 Dodge Ram pickup traveling eastbound and driven by Jonathan M. James, 32, of Spirit Lake, Idaho. The 16-year old juvenile and Jonathan James were transported by ambulance to Kootenai Health. The highway was blocked for approximately 2.5 hours. Investigation continues.

At approximately 11:41 AM on March 6th, 2019, Idaho State Police received a report of a blocking semi truck crash on US12 milepost 148.9, west of the Powell area. Christopher W Johnson, 34 year old male of Missoula, Montana, was driving a truck and trailer loaded with wood chips. Johnson was traveling westbound on US12 negotiating a right turn when the tractor and trailer laid over onto the driver's side. The roadway was completely blocked for four hours, and the roadway continues to be partially blocked during recovery of the trailer. Johnson was transported by ground ambulance to Syringa Hospital in Grangeville. He was wearing a seatbelt. Investigation and recovery of the trailer continues.

On March 5, 2019, at 5:21 p.m., Idaho State Police investigated a two vehicle fatality crash on US30 at milepost 195.4, near Buhl. Robin Parry, 37, of Hollister, was driving a 2006 Subaru Impreza westbound on US30. Dennis Norwood, 62, of Buhl, was driving a 2013 Toyota Tacoma pickup eastbound on US30 at milepost 195.4. Parry drifted over the center line and collided with Norwood head on. Norwood went off the right shoulder of the road. Parry's vehicle came to rest in the eastbound lanes of travel.


Parry succumbed to her injuries at the scene of the crash. Derek Haas, 50, of Hagerman was a passenger in Parry's vehicle. Haas and Norwood were transported by air ambulance to St. Alphonsus Regional Medical Center in Boise. Parry and Haas were wearing seatbelts. Norwood was not wearing a seatbelt.


The lanes of travel were blocked for approximately 2.5 hours for the investigation. Next of kin has been notified.

Gaylen W. Clayson, 65, of Firth was driving northbound in a 2005 International semi pulling a single trailer. Clayson exited the interstate using the off ramp. The semi and trailer overturned coming to rest in the roadway blocking the off and on ramp.

Clayson was transported by ground ambulance to Bingham Memorial Hospital, in Blackfoot. Clayson was wearing a seat belt.

The roadway was blocked for approximately four hours.

On Thursday, February 28, 2019, at 6:43 p.m., Idaho State Police investigated an injury crash on Yellowstone Avenue south of Alameda Road, in Pocatello.

Emily A. Faulkner, 27, of Richmond, UT, was stopped at the stoplight northbound on Yellowstone Avenue in a 2017 Mazda MZ3. Pocatello Police Officer Joel D. Greenway, 32, of Rigby, was following too close in his patrol car. Greenway hit Faulkner's vehicle from the rear causing a chain reaction with two other vehicles which were also stopped at the light. Those other vehicles and drivers are:

Sharon E. Obrien, 63, and her passenger, Stephen C. Obrien, 66, both of Blackfoot, driving a 2016 Subaru Forester

Gabrielle S. Tragis, 24, of Pocatello, driving a 2009 Ford Fusion

Faulkner was transported by ground ambulance to Portneuf Medical Center. All drivers and passengers were wearing seatbelts.

The northbound lanes on Yellowstone Avenue were blocked for approximately one hour.
