On October 30, 2016, at 2:35 a.m., the Idaho State Police investigated a single-vehicle, injury crash near N 100 W/W 100 N, in Jerome County.

Jorge Gonzalez-Hernandes, 37, of Jerome, was driving westbound on N 100 W in a 2001 Chevrolet Tahoe. The vehicle drove off the right shoulder and rolled.

Gonzalez-Hernandez was transported via air ambulance to St. Alphonsus Regional Medical Center in Boise. His passengers, Carmen Vidriesca Meza, 35, and Arturo Triana-Lara, 51, of Jerome, were not transported.

It is not known at this time if the occupants were wearing seatbelts.

On October 28, 2016, at approximately 8:41 p.m., Idaho State Police investigated a two vehicle injury crash on US95 at milepost 46, just south of Parma.

Ken Davetian, 46, of Glendale, CA., was traveling southbound on US95 in a 2006 Freightliner semi truck. Robert Dempsey, 39, of Caldwell, was traveling northbound on US95 in a 1992 Dodge Dakota pickup. Dempsey's vehicle crossed the center line and struck the semi head on.

Dempsey was transported by air ambulance to St Alphonsus in Boise. It is unknown if either driver was wearing a seat belt. The road was blocked for approximately six hours while crews investigated.

INCIDENT NARRATIVE: Mills was traveling east on US12 when he went onto the right shoulder. He cut across both lanes of travel and his vehicle began to roll, coming to a stop in the west bound lanes. Mills was transported to Clearwater Valley Hospital and then air lifted to St. Joes Medical Center. It is unknown if he was wearing a seatbelt. The road was blocked for three and a half hours. The investigation is continuing.

On Friday, October 28th, at approximately 4:43 p.m., the Idaho State Police investigated a two-vehicle fatality crash at Elmore Road and Southwest 1st Avenue, in Payette County. Byron Milburn, 37, of Caldwell, was southbound on Elmore Road in a 2011 Mercedes Benz Sprinter. Timothy Eicher, 23, of New Plymouth, was westbound on Southwest 1st Avenue in a 1998 International farm truck. Milburn failed to yield for the stop sign and was hit by the International truck.

Milburn was not wearing a seatbelt and succumbed to his injuries. Eicher was wearing his seatbelt. Next of kin has been notified.

On October 27, 2016, at approximately 5:34 p.m., the Idaho State Police investigated a rollover crash westbound Interstate 184 at milepost 3, near the Fairview exit. Kristan Selby, age 20, of Boise, was entering Interstate 184 from Fairview in a 2007 Mazda MZ3, when she tried to avoid a collision with a second vehicle. The second vehicle was trying to avoid a collision with a third vehicle. Selby then lost control of her vehicle, ran into the jersey barrier, and her vehicle overturned. Selby was transported by ground ambulance to St. Alphonsus Regional Medical Center in Boise.

Selby was wearing a seatbelt. The two left lanes of Interstate 184 were blocked for approximately half an hour while crews worked to clear the scene.

The Idaho State Police is requesting the public's help in locating the two other vehicles involved in the crash. One of the vehicles is described as a white four door sedan, possibly a Chevrolet Malibu, with an Idaho elk partial license plate of 9 and E. The second vehicle was described as a silver Saturn hatchback type of vehicle. If you were a witness or know any further information, please contact the Idaho State Police at 208-846-7500.

The crash is under investigation by the Idaho State Police.

On October 27, 2016, at approximately 5:02 p.m., the Idaho State Police investigated a single vehicle injury crash on Interstate 15 at milepost 93, near Blackfoot. Peggy Hill, 56, of Pocatello, was traveling southbound in a 2003 Chevy Trailblazer when she made an unsafe lane change, overcorrected, hit the guardrail and rolled into the median.

Hill was transported by ground ambulance to Portneuf Medical Center in Pocatello. She was not wearing a seatbelt. The crash is under investigation by the Idaho State Police.

INCIDENT NARRATIVE: Rodriguez was northbound on US95 when he slowed to avoid a deer. His load shifted, sending a 2300 pound ingot of lead onto the roadway. Tobias was southbound and struck in the ingot. This caused the front axle of Tobias' semi to be ripped out and sent the semi skidding on the roadway. The semi came to rest on the northbound side of the road, near the ditch. Tobias jumped out of the semi due to a fire, his load had shifted and he was struck by a truck he was hauling. The roadway was blocked for over 7 hours. Rodriguez was issued a citation for unsecure load.

Robert Gamble, 86, of Meridian, was traveling southbound in a 2005 Toyota Highlander when he crossed the centerline and left the northbound edge of the roadway striking a culvert and two trees.

Gamble was transported by air ambulance to Saint Alphonsus in Boise. Gamble was wearing a seatbelt. This crash is under investigations by the Idaho State Police.

Mcintyre's vehicle then spun around and struck Nelson's vehicle. Chandler and Nelson's vehicles then went off the embankment of the East Bound side of the road. Next of kin has been notified. On October 24, 2016, at approximately 11:45 pm, an officer with the Payette Police Department initiated a vehicle pursuit for a traffic violation. The pursuit began in Payette City, and then proceeded south into the City of Fruitland, where a Fruitland Officer deployed spike strips in an attempt to stop the suspect. The spike strips successfully deflated one tire on the suspect's vehicle, which slowed the speed of the vehicle, but did not bring it to a stop. The pursuit continued southbound on US Highway 95 into Payette County toward Interstate 84. The suspect entered the opposing lanes of traffic by turning onto I-84 going eastbound down the exit 3 westbound off-ramp.

Before coming to the end of the off-ramp, the suspect vehicle swerved and crashed into the guard rail and then turned around and accelerated back toward the pursuing officers. A Fruitland Police Department officer exited his vehicle and gave commands to the suspect. The suspect disobeyed the officer's commands and a confrontation ensued, which resulted in the Fruitland Police Department officer firing at the suspect ultimately striking him. The suspect's vehicle continued back onto US Highway 95 ultimately coming to a stop near the top of the off-ramp. Officers converged on the vehicle and began first aid and life saving measures on the suspect.

The suspect, a forty-six year old white male from Harrison Idaho was the sole occupant of the vehicle and is currently in stable condition at St. Alphonsus Regional Medical Center in Boise. The suspect's name is not being released until his family has been notified. The Idaho State Police have been asked to lead the investigation in the shooting.

Chief JD Huff of the Fruitland Police Department said, "This is a very serious situation and I can assure the citizens of the City of Fruitland and Payette County that we will work closely with the Idaho State Police to ensure a thorough and complete investigation is conducted."

On Monday, October 24, 2016, at 6:00 a.m., Idaho State Police investigated a single-vehicle, fatality crash on I-15 milepost 178, near Spencer.

Jennifer McIntosh, 27, of Bishop, California, was driving northbound on I-15 in a 1999 Ford pickup. McIntosh drove the pickup onto the right shoulder, then overcorrected. She then drove across the lanes into the median, where the pickup rolled.

McIntosh and her juvenile passenger were both wearing seatbelts. McIntosh succumbed to her injuries at the scene of the crash. Her passenger was transported via ground ambulance to Eastern Idaho Regional Medical Center in Idaho Falls.

The crash is under investigation by the Idaho State Police with assistance at the scene from Clark County Sheriff's Office.

On October 24, 2016 at approximately 10:45 am, the Idaho State Police investigated a two-vehicle, fatality crash northbound SH-34 at milepost 12, one mile south of Riverdale.

Ivan Mott, 44, of Weston, was southbound on SH-34 at milepost 12 in a 1999 Chevrolet Suburban. Robert Newswander, 49, of Hyde Park, Utah, was northbound on SH-34 at milepost 12 in a 2015 Ram pickup pulling a flatbed trailer. The Chevrolet crossed into the northbound lanes and struck the front driver's side of the Ram pickup.

Ivan Mott and his passenger Jonathan Mott, 18, of Weston, were both transported to Franklin County Medical Center in Preston by ground ambulance. Ivan Mott was wearing his seatbelt; Jonathan was not. Ivan Mott succumbed to his injuries at the hospital. Neither Newswander nor his passenger Ernesto Mariscal-Farias, 39, of Clifton, were transported. Both were wearing their seatbelts.

The road was partially blocked for approximately four hours. Next of kin has been notified.

The crash is under investigation by the Idaho State Police.

INCIDENT NARRATIVE: On 10/21/2016 at 9:00 PM the Idaho State Police was dispatched to investigate a Lewiston Police Fleet crash in the North 40 Parking Lot in Lewiston, Idaho. Janette Eng-Kennedy had turned west into the parking lot from Juniper Drive attempting to cut across the parking lot onto Thain Grade. Eng-Kennedy failed to stop at a stop sign and was struck by Krakalia with the Lewiston Police Department. Officer Krakalia and his passenger, Denise Donovan, Idaho Dept of Corrections Parole Officer sustained minor injuries. Eng-Kennedy was not injured. Eng-Kennedy was cited for failing to stop at a stop sign.

On Saturday October 22nd 2016, at approximately 8:43 p.m. the Idaho State Police investigated a three-vehicle injury crash on State Highway 77 at the intersection of State Highway 81 in Declo. Kevin Stocking, 43 of Carey, ID was southbound on State Highway 77 in a 2003 Honda Pilot. Sofia Lopez-Guadarrama, 43 of Declo, was eastbound on State Highway 81 in a 2003 GMC Yukon. Stocking pulled into the intersection in front of the Yukon and was struck on the passenger side. Stocking's vehicle then rolled and landed on a 2011 Toyota Yaris driven by Kenneth Weeks, 57 of Burley, which was northbound on State Highway 77.
Four passengers from Stocking's vehicle were transported by ground ambulance to Cassia Regional Medical Center in Burley. All drivers were wearing seatbelts.

On Saturday, October 22, 2016 at approximately 1:30 p.m., Idaho State Police investigated a single vehicle fatality crash on Notus road near Red Top road, in Canyon County.

Martha Booth, 65, of Parma was driving northbound on Notus road in a 1998 Dodge pickup. Booth drove the vehicle off the left shoulder, came back onto the roadway, where she overcorrected, and the vehicle rolled.

A minor child was ejected and succumbed to their injuries on scene. Booth was transported by air ambulance to St. Alphonsus in Boise. Neither occupant was wearing a seatbelt. Next of kin has been notified.

The road was blocked for approximately three and a half hours while crews investigated.

On Thursday, October 20, 2016, at 11:07am, Idaho State Police investigated a non-injury crash eastbound on I86 at mile marker 54, west of Chubbuck.

Victor R. Butler, 32, of Homestead, FL was driving eastbound on I86 in a 2017 Freightliner semi truck hauling a trailer. Butler attempted to use the emergency cross over and was struck by a 2016 Toyota Tundra, driven by Austin R. Webb, 37, of Idaho Falls, who was also traveling eastbound on I86.

Butler, Webb, and Butler's passenger, Hakeen A. Mixson, 33, of Florida City, FL, were all wearing their seatbelts. Both lanes of I86 were blocked for one and half hours while crews worked to clear the scene.

On Tuesday, October 18, 2016, at approximately 6:43 p.m., Idaho State Police investigated a two-vehicle crash in Canyon County.

Joseph Upchurch, age 35, of Nampa, was driving a 1997 Ford Taurus eastbound on US Highway 20/26 near the intersection of Farmway Road, near Caldwell. Upchurch crossed the center line and collided head-on with a 2002 Chevrolet Impala driven by Jorge Ruiz, age 38, of Nyssa, Oregon. Upchurch's vehicle then rolled and came to rest off of the north side of the road.

Upchurch was wearing a seat belt and transported to West Valley Medical Center in Caldwell; Ruiz was not transported. The highway was blocked, and traffic diverted onto county roads, for approximately two hours while crews worked to clear the scene.

This crash remains under investigation by the Idaho State Police.

Early this morning, October 18, 2016, the Idaho State Police responded to a crash at 385 E Highway 26, east of Shoshone. Weston Kirkpatrick, age 21 of Hamer, was driving westbound on US26 near milepost 170 when he went off the road on the left side. Kirkpatrick overcorrected, and his vehicle went back across the highway, off the road on the right side, and through a residential fence at 385 E. Hwy 26. The vehicle then rolled, coming to rest in the front yard of the residence. Kirkpatrick was ejected and died at the scene of the crash. He was not wearing his seatbelt. Notification has been made to next of kin.

Shortly after 6:30 a.m. today, October 17, 2016, the Idaho State Police responded to a series of crashes westbound on I86 at milepost 49, at the Rainbow Rd. overpass. Kim Sortor, age 54, of Pocatello, was driving westbound in a Chevrolet Sonic when she struck a moose in the roadway. Jaime Velasco, age 41, of Cedar City, UT was also westbound in a 1994 Honda Accord when he swerved to avoid the dead moose from Sortor's crash. Velasco's vehicle slid into the median where it overturned and struck the concrete pillar of the Rainbow Road overpass. Lashae Hamilton, age 20, of Springfield,ID was also westbound in a 2003 Subaru Outback when she struck the dead moose. Hamilton then lost control of her vehicle, which slid off the right shoulder of the road.

All occupants were wearing their seatbelts. Kim Sortor and Lashae Hamilton were not injured. Jaime Velasco and his passenger, Jesus Felix, age 38 and also of Cedar City,UT, were both transported by ground ambulance to Portneuf Medical Center in Pocatello. Westbound traffic on I86 was closed for approximately an hour, and traffic was diverted at the Rainbow Road exit.

On Friday, October 14,2016, at approximately 8:35 a.m., Idaho State Police investigated a single vehicle injury crash on US91 at milepost 97, near Blackfoot. Kirsi Wilcox, 20 of Idaho Falls, was driving 2011 Toyota Prius southbound on I15 near milepost 89, near Blackfoot. Wilcox drove into the median and between two bridges and then vaulted over the embankment. Wilcox landed on US91 below I15 and rolled, coming to rest on the railroad tracks.

Ahmetovic was transported by ground ambulance to West Valley Medical Center in Caldwell. Mcanallen and two juvenile passengers were transported by ground ambulance to St. Alphonsus in Boise. On October 6, 2016, at approximately 8:12 p.m., the Idaho State Police investigated a three vehicle fatality crash eastbound Interstate 84 at milepost 175, east of Twin Falls. Minnie Vangorder, age 24, of Elko, NV, was driving westbound Interstate 84 at milepost 175 in a 2013 Toyota 4Runner, when she struck the rear of a 1994 Ford Ranger pickup driven by Randy Hansen, age 63, of Jerome, ID. Hansen's vehicle drove through the median and into the eastbound lanes of Interstate 84, where his vehicle struck the front of a 2007 Toyota Sienna driven by Sheri Christensen, age 53, of Soda Springs, ID. Christensen and Hansen were transported by ground ambulance to St. Luke's Magic Valley Medical Center in Twin Falls, ID. Hansen succumbed to his injuries. Notifications to family have been made.

The right lane of eastbound Interstate 84 was blocked for approximately four and a half hours while crews worked to clear the scene. Vangorder and Christensen were wearing their seatbelts, and Hansen was not wearing a seatbelt. The crash is under investigation by the Idaho State Police.

On Thursday, October 6, 2016, at approximately 4:19 p.m., the Idaho State Police investigated reports of multiple crashes near Rigby on U.S. Highway 20. The area experienced some severe weather including rain and hail, and drivers failed to slow for the inclement conditions. Initially, a driver of a 2005 Toyota van lost control, and the vehicle rolled into the median. Other motorists in the area, failing to slow for the road conditions and for the existing crash, resulted in a total of three reportable crashes and several non-reportable slide-offs and minor crashes.

As we approach the time for further winter weather conditions, Idaho State Police remind and encourage motorists to slow their driving for the present weather and road conditions, and to be mindful of any existing traffic through the area.


The Idaho State Police were assisted by Jefferson County Sheriff's Office and the Idaho Transportation Department in handling the reports and traffic control.

On Tuesday, October 4, 2016, at 7:45 a.m., the Idaho State Police investigated a single-vehicle, fatality crash on Chicken Dinner Road, near Homedale Road, in Canyon County.

At approximately 5:30 a. m., Austin Glenn, 22, of Marsing, was driving northbound on Chicken Dinner Road in a 2000 Subaru Legacy. The Subaru went off the right shoulder where it rolled and came to rest in a field.

Glenn was ejected from the vehicle and succumbed to his injuries at the scene of the crash. Glenn was not wearing a seatbelt.

The crash is under investigation by the Idaho State Police.

On Tuesday, October 4, 2016 at approximately 2:43 am, Idaho State Police investigated a two vehicle injury crash, southbound Interstate 15 at milepost 62, near Pocatello. Frank Talbot, 80, of Roy, Utah, was driving a 2003 Ford Windstar Van. Talbot was driving the wrong way, northbound in the southbound lanes of travel. Talbot's vehicle struck a 2007 GMC Yukon driven by a minor. Both vehicles entered the median.

Talbot was transported by ground ambulance to Portneuf Medical Center. He was not wearing a seatbelt. The left lane of travel was blocked for approximately one hour while crews investigated.

John Hadam, 85, of Eden, was driving northbound on N 3800 E in a 1972 Mack truck and struck a boom truck owned by the Twin Falls Highway District. The boom truck was parked facing south in the northbound lane for workers to trim trees.

Daniel Milliken, 33, of Filer, was in the boom. Kody Craner, 43, of Twin Falls and Arthur Clark, 43, of Kimberly were standing behind the boom truck which was pushed into them.

Milliken was transported via air ambulance and Craner was transported via ground ambulance to St. Lukes Magic Valley Medical Center in Twin Falls.

On October 2, 2016, at approximately 12:00 noon, Idaho State Police investigated a single vehicle crash in Twin Falls County, between Filer and Buhl.

Charles M. Cantrell, 32, of Buhl was driving a 2016 Ford F150 pickup westbound on 4100 N near 2150 E. Cantrell drove left of the center painted dividing line and struck a bridge abutment.

Passengers in the vehicle were Mackensie A. Cantrell, 34, also of Buhl, and two minor children. Mackensie and the minors were wearing age-appropriate safety restraints. Charles was not wearing a seatbelt. All occupants were transported by ground ambulance to St. Luke's Magic Valley Medical Center in Twin Falls.

On October 2, 2016, at approximately 6:45 P.M., Idaho State Police investigated a two-vehicle injury crash on State Highway 39 at milepost 11, just south of Aberdeen.

Marcelino Albino, 25, of Fort Hall, was driving northbound on SH39 in a 2004 GMC Envoy. Albino attempted to make a left turn and collided with a 2005 Pontiac Grand Prix that was traveling southbound on SH39. The Pontiac was driven by Mckayla Elquezabal, 21, of American Falls.

Elquezabal and her passenger Dustin Brown, 23, of Twin Falls, were transported by ground ambulance to the Portneuf Medical Center in Pocatello. A juvenile was also transported to the hospital.

Albino was taken into custody by the Idaho State Police and transported to the Bingham County Jail.

Alcohol was involved.

State Highway 39 was blocked for approximately one hour.

On October 2, 2016, at approximately 4:47 P.M., Idaho State Police investigated a one-vehicle property damage crash on northbound Interstate 15 at milepost 58, near Inkom.

Zhidao Li, 27, of China, was driving southbound on I15 in a silver 2016 Dodge Caravan. Li drove off the right shoulder, struck the guard rail, drove back across the southbound lanes of travel and through the median. Li overturned the vehicle and came to rest in the right lane on the northbound side of I15. All occupants were wearing a seat belt.

Li's passenger Pan Xiaochan, 30, of China, was transported by ground ambulance to the Portneuf Medical Center in Pocatello.

The northbound right lane of travel was blocked for approximately one hour.

On October 2, 2016, at approximately 12:00 noon, Idaho State Police investigated a single vehicle crash in Twin Falls County, between Filer and Buhl.

Charles M. Cantrell, 32, of Buhl was driving a 2016 Ford F150 pickup westbound on 4100 N near 2150 E. Cantrell drove left of the center painted dividing line and struck a bridge abutment.

Passengers in the vehicle were Mackensie A. Cantrell, 34, also of Buhl, and two minor children. Mackensie and the minors were wearing age-appropriate safety restraints. Charles was not wearing a seatbelt. All occupants were transported by ground ambulance to St. Luke's Magic Valley Medical Center in Twin Falls.



The crash investigation is continuing.
