On February 28, 2016, at approximately 10:49 a.m., Idaho State Police investigated a non-injury crash involving two semi trucks. Brad Rollins, 35, of Caliente, Nevada, was driving westbound on Interstate 84, near milepost 108, in a 2013 Volvo tractor pulling a single trailer. Rollins fell asleep at the wheel and rear ended a second Volvo tractor and single trailer, driven by Daniel Weaver, 56, of Salem, Oregon.

Both drivers were wearing seatbelts. The right lane of westbound travel was blocked for approximately 2.5 hours while the scene was cleared.

At 9:15 a.m. this morning, February 27, 2016, the Idaho State Police investigated a two-vehicle non-injury crash at the intersection of University Blvd. and South Yellowstone Highway in Rexburg. Officer Wright, an officer with the Rexburg Police Department, was responding to a call in a marked patrol vehicle, driving eastbound on University Blvd. He entered the intersection of South Yellowstone Highway with his emergency lights activated. Kerrey Johnson, age 38, of Idaho Falls, was northbound on South Yellowstone Highway in a Mitsubishi Galant and entered the intersection. The two vehicles collided in the center of the intersection. The investigation is ongoing.

On Friday, February 26, 2016, at approximately 4:35 p.m., Idaho State Police investigated a three vehicle crash, westbound on Interstate 84 at the Garrity off ramp.

Casey Vanblaricom, 30, of Post Falls, was attempting to exit at the Garrity off ramp in a 2004 Hyundai Tiburon, when traffic came to a stop in front of him. Vanblaricom's vehicle collided with a 1995 Chevy S10 pickup, driven by Cameron Hand, 17, of Nampa. Upon impact, Hand's vehicle traveled across the westbound lanes of the interstate, and collided head on with a 2016 Ford F150 pickup, driven by Arlo Decker, 66, of New Plymouth.

Hand was transported by ground ambulance to St. Alphonsus in Nampa. The passengers of the Ford were transported by ground ambulance to St. Luke's in Meridian. All occupants were wearing their seatbelts. Two lanes were blocked for approximately 45 minutes while crews worked to clean up the scene. The crash is under investigation by Idaho State Police.

Ronald Cardwell, 36, of Rancho Santa Fe, California has been identified of the driver of the 2006 Jeep Commander that was involved in this crash. Family notifications have been made.

On February 25, 2016, at approximately 6:20 p.m., Idaho State Police investigated a two-vehicle crash on US 20 Milepost 376, between Ashton and Island Park, ID.

Matthew A. Martin, 36, of Salt Lake City, UT., was driving northbound in a 2016 Freightliner pulling a 2015 Great Dane trailer, when a southbound vehicle drove left of the center line. The vehicles collided head-on, and both vehicles came to rest off of the southbound shoulder of the roadway.

Martin was wearing a seatbelt. He was treated and released at the scene of the crash. The driver of the southbound vehicle succumbed to his injuries at the crash scene. The name is being withheld at this time until the family has been notified.

On Wednesday February 24, 2016, at approximately 5:24 pm, Idaho State Police investigated an injury crash westbound on US26 at milepost 376, near Swan Valley, ID.

A 1995 Peterbilt tow truck was partially in the westbound lane of travel with the emergency lights activated, preparing to tow a vehicle. John J. Quick, 39, of Rexburg, was driving westbound on US26 in a 1999 Dodge Durango. Quick collided into the rear of the tow truck. Upon impact the Durango then traveled off the south side of the roadway and down and embankment into a culvert.

Quick was extricated from the vehicle and transported by ground ambulance to Eastern Idaho Regional Medical Center in Idaho Falls. He was not wearing his seatbelt. The driver of the tow truck, Billy J. Gerdes, 33, of Idaho Falls was not injured.

At approximately 11:22 p.m. on Monday, February 22, 2016, Idaho State Police investigated a two vehicle crash involving a FedEx truck, westbound on Interstate 84 near milepost 97, just east of Mountain Home.


Tayler Kerns, 22, of Haines, Oregon, and two passengers Thomas Newbill, 29, of Durham, Californa and Christopher Patrick, 23, of Baker City, Oregon, were stationary on the westbound right shoulder of the Interstate in a 2006 Dodge pickup pulling a trailer carrying a 2007 Dodge pickup. Travis Trowbridge, 27, of Boise, was travelling westbound in a 2015 Volvo FedEx tractor hauling three trailers. The commercial vehicle struck the trailer with the Dodge pickup on the right shoulder. The second and third fedex trailers overturned and the vehicle came to rest in the median. The third trailer caught fire.

The eastbound and westbound lanes were blocked for approximately five and a half hours while crews worked to clear the scene. The eastbound lanes have been open for approximately two hours while the westbound side remains blocked at this time for cleanup.

No one was injured in the crash. Trowbridge was wearing his seatbelt at the time of the collision.This crash is under investigation by the Idaho State Police.

On Saturday, February 20, 2016, at 7:50 a.m. the Idaho State Police investigated a two-vehicle crash near Springfield.

Santiago Nava Sanchez, 64, of Aberdeen was driving southbound on State Highway 39 in a 1997 Ford F250 pickup. Donald Raines, 55, of Draper, Utah, was following behind in a 2015 Peterbilt hauling potatoes. Raines attempted to pass Nava Sanchez. While Raines was passing, Nava Sanchez made a left hand turn and was struck by Raines. The pickup left the roadway on the right shoulder and overturned. All occupants were wearing seatbelts; no one was injured. State Highway 39 was blocked for approximately one hour.

This crash is under investigation by the Idaho State Police.

On February 20, 2016, at approximately 9:03 a.m., Idaho State Police investigated an injury crash at North Midland Boulevard and West Railroad Street in Nampa, Canyon County, Idaho. Vanessa Guillen, 25, of Notus, was traveling south on Midland Boulevard in a 2002 Pontiac Grand Am. Guillen drove around the railroad crossing arms and was struck by an eastbound Union Pacific train. The Pontiac spun off the road, striking a large electrical box. The crossing arms were down and active at the time of the crash. Guillen was not wearing a seatbelt and was transported by ground ambulance to St. Alphonsus Medical Center in Boise. The road was blocked for approximately three hours.

This crash remains under investigation by the Idaho State Police.

On Thursday, February 18, 2016, at approximately 10:30 P.M., Idaho State Police investigated a one-vehicle roll over crash on US30 near milepost 367, in Bannock County.

Sonia McGaha, 51, of Batesville, Arkansas, was driving eastbound on US30 in a 1998 Peterbuilt. McGaha's vehicle went off the right shoulder causing the vehicle and the attached trailer to overturn onto their sides. McGaha was wearing a seatbelt.

The road was not blocked due the crash, however, the right eastbound lane will be blocked for an extended period as crews work to recover the semi and trailer.

On Sunday, February 14, 2016, at approximately 10:00 a.m. Idaho State Police investigated a jackknifed semi-truck south of Pocatello on Interstate 15. Driver Sean Mullins, 48, of Magna, Utah was travelling southbound in a 2013 Freightliner pulling an empty trailer, when he lost control of the vehicle near milepost 63. The semi-truck jackknifed, coming to a stop on the median side of the interstate, blocking the left southbound lane of travel.

Both lanes were blocked for approximately one and a half hours while the crash site was cleared.
Mullins was wearing a seatbelt.
This crash is under investigation by the Idaho State Police.

At approximately 1:30 p.m. this afternoon, February 13, 2016, Idaho State Police investigated a two-vehicle crash at the intersection of W 2000 S and State St. in Preston. Brock Bitton, age 36, of Logan, UT was driving southbound on State St. in a Mitsubishi Galant. Marie Mitchell, age 32 of Lewiston, UT was northbound in a Ford Fusion. Bitton crossed over the divider and struck Mitchell. Both drivers were wearing seatbelts. Bitton succumbed to his injuries at the scene of the crash. Mitchell was transported by ground ambulance to Logan Regional Hospital in Logan, UT. State Street was blocked for approximately four hours. The investigation is ongoing with assistance from Franklin County Sheriff's Office. Next of kin has been notified.

Shortly before 7 p.m. this evening, February 10, 2016, a two-vehicle crash claimed the life of one motorist. Gayle Giles, age 73, of Shelley, was eastbound on US26 at milepost 291, about 14 miles west of Blackfoot. Giles was driving at a slow speed and was straddling the dividing line. Lynn Marlow, age 62, of Rigby, was also eastbound. Marlow struck Giles from behind. Giles was transported to Bingham Memorial Hospital in Blackfoot, where she succumbed to her injuries.

INCIDENT NARRATIVE: Duman was Southbound on US95 at MP 310 traveling in the left lane when he failed to see Litteneker driving next to him in the right lane. Duman began to move into the right lane, causing Litteneker to take evasive actions by driving onto the shoulder and accelerating. Dumans semi made contact with Littenekers rear quarter panel, causing Litteneker to lose control, crossing over the left lane in into the grassy median between the highway and the Lower Port of Entry. Littenekers vehicle struck a lamp post and rolled coming to rest in the median. Duman was cited for inattentive/careless driving.

On February 10, 2016, at approximately 6:35 a.m., Idaho State Police investigated a single vehicle rollover crash on US Highway 91 at Cemetery Road, in Fort Hall, ID. Logan Eskelsen, age 18 of Blackfoot, ID, was traveling southbound in a 1997 Jeep Grand Cherokee. Eskelsen swerved to miss a dog in the road, lost control of his vehicle, and rolled his vehicle into a ditch. Eskelsen was transported by private vehicle to Bingham Memorial Hospital in Blackfoot.

Eskelsen was wearing a seatbelt at the time of the crash. The crash is under investigation by Idaho State Police.

On Sunday, February 7, 2016, at approximately 10:00 P.M., the Idaho State Police investigated a two-vehicle injury crash on SH55 in Caldwell near Montana Ave.

Elvis Mahurin, 47, of Kuna, was traveling northbound in a 2006 Dodge 1500 pickup.
Cory Johnson, 28, of Caldwell, was traveling southbound in a 2004 Suzuki Verona.

Mahurin made a U-Turn and was struck by Johnson. Johnson's vehicle rolled and came to rest on its top.

All the occupants of Johnson's vehicle were transported by ground ambulance to St Alphonsus Medical Center in Boise. Both drivers were wearing their seatbelts, while passengers in Johnson's vehicle were not.

The roadway was blocked for approximately two hours while the wreckage was removed.

On February 5, 2016, at approximately 6:38 P.M., Idaho State Police investigated a two vehicle fatality crash northbound on SH55 at milepost 6, near Marsing.

Jarom Empey, 21, of Mesa, WA was driving northbound in a 2004 Ford pickup. Stephanie Frazier, 28, of Marsing, was driving soutbound in a 2011 Kia Optima. Empey drove across the center line, and collided head-on with Frazier's vehicle.

Also in Frazier's vehicle were Christopher Frazier, 35, of Marsing, and two minor children. Christopher Frazier and one child succumbed to their injuries at the scene. Stephanie Frazier and the second child were transported by ground ambulance to St. Alphonsus Regional Medical Center in Boise. Empey was transported by ground ambulance to West Valley Hospital in Caldwell. All subjects were wearing seatbelts.

Southbound lanes and one northbound lane were blocked for approximately 4 hours. The crash is under investigation at this time.

On February 5, 2016 at 3:01 pm, Idaho State Police investigated an injury crash westbound on I84 at mile marker 163, west of Jerome.

Harold T. Mays, 45, of Hagerman, was traveling westbound on I84 in a 2015 Toyota Tacoma. Mays slowed to avoid hitting a dog in the roadway. His Toyota was struck from behind by a 2000 Ford F350 driven by Rocky E. Scarrow, 39, of Jerome. Upon impact the Ford rolled into the median.

Mays, Scarrow, and a passenger in Scarrow's vehicle were all transported by personal vehicle to local hospitals. Mays was wearing his seatbelt. Scarrow and his passenger were not.

The left lane of travel was blocked for approximately one hour.

On Thursday, February 4, 2016, at approximately 10:25 a.m., Idaho State Police investigated an injury crash in the 400 block of Caldwell Boulevard, in Nampa.

Osbaldo Gonzalez, age 36, of Caldwell, was driving a 1991 Buick Park Avenue eastbound on Caldwell Boulevard. Thu Tran, age 28, of Nampa, was driving westbound in a 2002 Lexus sedan. As Gonzalez was turning left into a business at 408 Caldwell Boulevard, he failed to yield to oncoming traffic and collided with Tran's vehicle. Tran lost control of her vehicle, ran off the road, and struck multiple unoccupied vehicles in a car dealership parking lot.

Tran was transported by ground ambulance to St. Luke's in Nampa. The crash remains under investigation by the Idaho State Police.

The Idaho State Police is asking for the public's assistance regarding a pedestrian collision which occurred on 2/2/16, just after 3:30 pm on East Selle Road one half of a mile east of US 95 in Bonner County, Idaho. Anyone who saw the crash, or any pedestrians on this portion of Selle Road prior to 3:30 pm, who have not already given a statement to law enforcement, are asked to call 208-772-6055. The Idaho State Police would like to thank those citizens who remained on scene and rendered aid to the pedestrian.

INCIDENT NARRATIVE: Flasinski was stopped along the roadway doing maintenance on his semi tractor trailers in the snowy conditions on Lookout Pass. Fitzthum was traveling eastbound Interstate 90 at milepost 73.3 when the trailers of his semi impacted the trailers of Flasinski. Flasinski was struck by the movement of his own trailers upon impact from Fitzthums trailers. Flasinski succumbed to his injuries at the hospital. Next of kin has been notified.
