On Monday, August 31, 2015, at approximately 6:14 p.m., the Idaho State Police investigated a non-injury crash eastbound I-84 near milepost 70.5, near the Mayfield exit in, Ada County.

Ronald V. Nelson, 72, of Nampa, was driving a 1997 Dodge Ram pickup, pulling a camp trailer and a boat trailer with a boat on it. Nelson was driving eastbound on I-84 in the right lane. Nelson lost control, the camper turned on its side, the boat trailer turned over ejecting the boat. The right lane of travel was blocked for approximately two hours and forty minutes.

The crash is under investigation by Idaho State Police.

On Monday, August 31, 2015, at approximately 1:30 p.m., the Idaho State Police investigated a single vehicle verses a building crash at 110 E. Main St. in Hagerman, ID.

Maryann Poole, 49, of Hagerman, was driving a 2004 Jeep Cherokee westbound on Main St. Poole turned south into the Hagerman Police Department parking lot. Poole's foot slipped off the brake and onto the accelerator. The vehicle collided with the Hagerman Police Department building.

Poole was transported by ground ambulance to St. Luke's Magic Valley Medical Center in Twin
Falls. Poole was wearing a seat belt.

This crash is under investigation by the Idaho State Police.

On Saturday, August 29, 2015, at approximately 7:45 p.m., the Idaho State Police investigated a two-vehicle, injury crash on US Highway 20 at milepost 369, near Harriman State Park.

Brandon Morris, 34, of Gallatin Gateway, Montana, was driving eastbound on US 20 in a 2003 Subaru Forester. James Papworth, 62, of Rexburg, was driving westbound in a 2013 Chevrolet pickup towing a utility trailer. The Subaru crossed the center line and struck the pickup on the driver's side rear quarter panel and the utility trailer. All vehicles came to rest in a pullout on the side of the roadway.

Morris was not wearing a seatbelt and was extricated from his vehicle. He was transported via ground ambulance to Eastern Idaho Regional Medical Center in Idaho Falls. Papworth and his passengers were wearing seatbelts and were not transported. There was no lane blockage.

The crash is under investigation by the Idaho State Police.

On August 29, 2015, at 5:20 a.m., the Idaho State Police investigated a single-vehicle, fatality crash southbound I-15 at milepost 93, near Blackfoot. Gonzalo Lopez-Vasquez, 32, of Wimauma, Florida, was traveling southbound in a 2001 Ford Explorer when he went off the right shoulder, overcorrected, and rolled into the median, where the vehicle came to rest on its roof. Lopez-Vasquez's passenger, Meso Reymieri, 36, of Guatemala, was ejected from the vehicle, and succumbed to his injuries at the scene. Lopez-Vasquez was located a half mile from the crash, and was transported by ground ambulance to Bingham Memorial.

Lopez-Vasquez was arrested and charged with Leaving the Scene of a Fatality Crash and Vehicular Manslaughter. Lopez was wearing a seatbelt; Reymieri was not wearing a seatbelt. Notifications have been made. This crash is under investigation by the Idaho State Police.

On Friday, August 28, 2015, at 7:25p.m., the Idaho State Police investigated a two vehicle injury crash northbound on SH28 at milepost 51, near Lone Pine. John Steele, 69, of Salmon, was driving a 2013 Honda ATV northbound on the right side road edge, crossing a bridge. Cajsa Allen, 40, of Salmon, was driving a 2015 Toyota Sienna Van, also northbound.

Allen moved over into the southbound lane to go around Steele. As Allen passed, Steele turned left running into the side of Allen's vehicle. Steele was ejected off the ATV and landed on the highway. Allen was air lifted to Eastern Idaho Regional Medical Center. Allen was not wearing a helmet at the time of the crash. The Idaho State Police was assisted by the Clark County Sheriff's Office.

The crash is still under investigation at this time.

On Thursday, August 27, 2015, at approximately 12:45 p.m., the Idaho State Police investigated a two-vehicle injury crash on Interstate 84 at milepost 151, west of Wendell.

Jordan Phillips, 28, of Boise, was driving eastbound in a 1997 Mercedes passenger car. Pinchuk Oleksandr, 32, of Orangevale, California, was also eastbound in a 2011 Volvo semi truck. The Mercedes struck the back of the semi.

Phillips passengers, Nikoa Phillips, 32, of Boise, and an infant, were transported via ground ambulance to North Canyon Medical Center in Gooding, Idaho. All occupants were wearing seatbelts. There was no blockage due to the crash.

The crash is under investigation by the Idaho State Police.

On Wednesday, August 26, 2015, at approximately 9:02 a.m., the Idaho State Police investigated a single-vehicle, injury crash northbound SH-51 at milepost 27.3 in Owyhee County.

Cy Garner, 40, of Meridian, was northbound on SH-51 near Riddle in a 2004 Honda Accord. The Accord went off the right shoulder of the road on a curve, overcorrected, went off the left shoulder and rolled. The vehicle came to rest off the left side of the roadway.

Garner was transported to the St. Alphonsus Regional Medical Center in Boise by air ambulance. Garner was wearing his seatbelt. The road was blocked for approximately three-and-a-half hours.

The crash is under investigation by the Idaho State Police.

On Wednesday, August 26, 2015, at approximately 1:26 p.m., the Idaho State Police investigated a single-vehicle, injury crash eastbound I-84 near milepost 32.

Adrianna Guevara, 23, of Caldwell was westbound on I-84 near milepost 32 in a 1998 Honda Accord. The Honda left the roadway on the median side, rolled, and came to rest in the eastbound lanes. Guevara was ejected from the vehicle.

It is unknown if Guevara was wearing her seatbelt. She was transported to the St. Alphonsus Regional Medical Center in Boise by ground ambulance. There was partial blockage both westbound and eastbound for approximately one-and-a-half-hours.

The crash is under investigation by the Idaho State Police.

Bonners Ferry Man Sentenced for Violent ATM Theft in McCall and String of Prior ATM Larcenies across the U.S.

BOISE — Matthew Taber Annable, 40, formerly of Bonners Ferry, Idaho, was sentenced Tuesday to 120 months in prison for conspiracy to commit bank larceny, and aiding and abetting bank larceny by use of a dangerous weapon, U.S. Attorney Wendy J. Olson announced. Annable pleaded guilty to the charges on April 20, 2015.

According to the plea agreement, on January 10, 2014, Annable and his co-defendant, Nathan Paul Davenport, aiding and abetting each other, broke into and stole cash from an automated teller machine (ATM) located at the Idaho First Bank in McCall, Idaho. They stole a Ford truck in McCall, which Davenport used to pull the door off the ATM. When Davenport was escaping with the proceeds, he twice stopped the stolen truck and fired a Ruger .223 caliber assault rifle at pursing McCall Police Department officers. The officers’ vehicles were hit numerous times, but no one was injured. Davenport and Annable communicated over an open cell phone connection during the larceny and shootings and then met up after Davenport evaded the pursuing McCall Police officers. They moved the firearms, tools, and stolen ATM money from the stolen Ford to their own vehicle and escaped. However, on January 12, 2014, Davenport and Annable were arrested in Utah. When arrested, Davenport was carrying a bag with the proceeds of the McCall ATM larceny, approximately $27,000, and Annable was carrying approximately $11,000 from a prior larceny.

The McCall shooting and larceny were the final crimes in an ATM larceny spree that stretched from October, 2012, through January, 2014, through Florida, Texas, Colorado, Wyoming, Utah and other states. These larcenies included two committed against Idaho Banking Company in Boise and Meridian on January 5, 2014.

In addition to imposing a ten-year prison term, Senior U.S. District Judge Edward J. Lodge ordered that Annable serve five years of supervised release after his prison term, and pay restitution to victims, primarily the affected banks, of $527,715.15. Judge Lodge also ordered forfeiture of any proceeds of the crimes. Davenport, was sentenced on June 22, 2015, to a prison term of 217 months. The sentences for both men will be offset by 12 months and five days already spent in custody for related charges in Wyoming, and credit for time since their detention in Idaho on these charges.

The case was investigated by the Federal Bureau of Investigation, the Idaho State Police, the Valley County Sheriff’s Office, and the McCall Police Department.

On August 24, 2015, at approximately 5:22 p.m., Idaho State Police investigated a two-vehicle crash on Interstate 15, near mile marker 99. Julie Gustafson, 41, of Mesa, AZ, was driving northbound on I15 in a 2005 Acura TSX when she was struck in the rear by a 2013 Toyota Avalon, driven by Susan Horwith, 70, of Idaho Falls. The Acura went through the median and across the southbound lanes of I15, finally coming to rest on the right shoulder. The Toyota went off the right shoulder of the northbound lanes and went through a fence before rolling. Horwith was not wearing her seatbelt and was ejected from the vehicle. She succumbed to her injuries at the scene.

Gustafson was transported by ground ambulance to Bingham Memorial Hospital. Gustafson was wearing a seatbelt. Next of kin has been notified.


This crash is under investigation by the Idaho State Police.

On Sunday, August 23, 2015, at approximately 9:00 a.m., the Idaho State Police investigated a fatality crash on US Highway 95 at milepost 158, west of New Meadows. Kristen Blehm, 39, of Redding, California, was traveling southbound on US-95 in a 2014 Subaru Outback. Jeffrey Church, 44, of Fruitland, was traveling northbound in a 2011 Chevy pickup. Blehm's vehicle struck a deer. The deer then struck Church's vehicle, and went through the passenger side of the windshield.

Church and his back passenger, Hilda Nuttman, 88, of Cottonwood, were transported by ground ambulance to St Luke's McCall Medical Center. Church's front passenger, Gail Church, 67, of Fruitland, succumbed to her injuries at the scene. Blehm was not injured. Seatbelts were worn by all parties.

This crash is under investigation by the Idaho State Police.

On August 22, 2015, at approximately 1:45 p.m., Idaho State Police investigated a two-vehicle crash northbound on Interstate 15, at mile marker 101.5. Daniel Summers, 22, of Blackfoot, was driving northbound on I15 in a 2006 Hyundai Elantra when he swerved into the left lane to avoid running into a 2003 Peterbilt semi, driven by Brent Cooper, 62, also of Blackfoot. As a result of the lane change, Summers lost control of his Elantra and struck the driver side of the Peterbilt. After striking the Peterbilt, the Elantra slid off the left shoulder. As a result of the impact the Peterbilt slid off the left shoulder, overturned, and came to rest on its side in the median.

On August 21, 2015, at 10:06 p.m., Idaho State Police investigated a one vehicle rollover crash on Interstate 15 at milepost 133, near Roberts, Idaho.

Hunter J. McKinney, 15, of Great Falls, MT, was driving southbound on Interstate 15 in a 2006 Nissan Pathfinder. McKinney drove off the left side of the road and struck the guardrail. The vehicle then rolled and came to rest facing north in the right southbound lane.

Hunter and his passengers, Dustin J. McKinney, 36, Paige D. McKinney, 28, and two juveniles, all of Great Falls, MT were transported by ground ambulance to Eastern Idaho Regional Medical Center in Idaho Falls, ID. Another juvenile passenger was transported by air ambulance to Eastern Idaho Regional Medical Center in Idaho Falls, ID. All occupants were wearing seatbelts.

Southbound lanes were blocked for approximately two hours while crews worked to clear the scene.

This crash remains under investigation by the Idaho State Police.

On Friday, August 21, 2015, at approximately 12:27 p.m., the Idaho State Police investigated a two-vehicle, injury crash northbound SH-39 at milepost 43.5, five miles north of Pingree.

Paul Gallegos, 74, of Pingree, was driving northbound on SH-39 in a 2013 Kia Optima. Ashlyn Key, 20, of Blackfoot, was driving eastbound on 100 S in a 1998 Honda Accord. Key was stopped at the stop sign at the intersection of 100 S and SH-39. When she proceeded into the intersection the Kia struck the Honda on the passenger side.

Both vehicles came to rest off the roadway; there was no road blockage. All occupants were wearing their seatbelts. Gallegos was transported by ground ambulance to Portneuf Regional Medical Center in Pocatello, ID.

The crash is under investigation by the Idaho State Police.

JEROME - The Idaho State Police will continue to conduct "Adopt a Crossing" an enhanced traffic enforcement operation at railroad crossings in Twin Falls and Jerome counties. The increased patrols began in July and will continue through September 12, 2015. The operation was made possible with grant funding provided by the Idaho Department of Transportation?s Office of Highway Safety and Operation Lifesaver.

"The goal of this special enforcement operation is to increase public awareness of the potential dangers that exist at highway railroad intersections and to eliminate driver actions that can have tragic consequences," said Idaho State Police Sgt. Mark Donahue. "It is our hope that by informing the public that we are focusing on enforcing the traffic laws that pertain to these rail crossings that they always be more careful when approaching them."

The "Adopt a Crossing" enforcement operation will pick a rail crossing within their jurisdiction and focus on enforcing the laws that pertain to that crossing.

Thus far in 2015, there have been four vehicle/train collisions in Idaho. In 2014, there were 20 vehicle/train collisions resulting in one fatality along with two pedestrians who were killed while on train tracks.

Troopers remind drivers that they can avoid a citation, and more importantly injuries or death, by following these safety tips:

- Stay alert around railroad tracks. No texting, headphones or other distractions that would prevent you from hearing an approaching train; never mix rails and recreation.

- Always expect a train at each highway-rail intersection. Freight trains don't travel at fixed times, and schedules for passenger trains change.

- All train tracks are private property. Never walk on tracks; it's illegal trespass and highly dangerous. By the time a locomotive engineer sees a trespasser or vehicle on the tracks it's too late. It takes the average freight train traveling at 55 mph more than a mile - the length of 18 football fields - to stop. Trains cannot stop quickly enough to avoid a collision.

- The average locomotive weighs about 400,000 pounds or 200 tons; it can weigh up to 6,000 tons. This makes the weight ratio of a car to a train proportional to that of a soda can to a car. We all know what happens to a soda can hit by a car.

- Trains have the right of way 100% of the time over all pedestrians and vehicles, even emergency vehicles.

- A train can extend three feet or more beyond the steel rail, putting the safety zone for pedestrians well beyond the three foot mark. If there are rails on the railroad ties always assume the track is in use, even if there are weeds or the track looks unused.

- Trains can move in either direction at any time. Sometimes their cars are pushed by locomotives instead of being pulled.

- Today's trains are quieter than ever, producing no telltale "clackety-clack." Any approaching train is always closer, moving faster, than you think.
Remember to cross train tracks only at designated pedestrian or roadway crossings, and obey all warning signs and signals posted there.

For over twenty-five years the Idaho State Police and Idaho Operation Lifesaver have partnered to eliminate car verus train collisions, which result in preventable deaths and injuries.

MERIDIAN - Idaho State Police is asking for anyone that witnessed the incidents leading up to a crash that claimed the life of a Garden City man on I-84 east of Boise early Sunday morning to contact them about what they may have witnessed.

On Sunday, August 16th at 3:52 a.m., troopers responded to a fatal crash on eastbound I-84 at milepost 68, near the Port of Entry east of Boise. Winters Wareagle, 23, of Garden City was hit and killed when Patrick Musgrove, 28 of Meridian who was driving a 2003 Toyota Maxtrix ran over Wareagle who was lying on the pavement in the right lane of travel.

MERIDIAN - Idaho State Police are asking for the driver of a pickup that may have been witness to events leading up to a crash early Wednesday morning on Robinson Road at Locust Lane that killed a motorcycle rider to contact them.

The crash occurred at around 6:00 a.m. when Sharalyn Rose, 28, of Meridian, was eastbound on Locust Lane in a 2001 Chevrolet Lumina and Paul Mackrill, 44, of Nampa, was southbound on Robinson Road on a 2009 Yamaha WR450F motorcycle. Rose was stopped at stop sign on Locust Lane and then pulled into the intersection. It was then when Mackrill's Yamaha struck the Lumina on the driver's side.

The driver of a cement truck who witnessed the crash told troopers that there was a silver 2006 to 2008 model year Dodge diesel pickup truck with an extra fuel cell in the bed of the pickup that was traveling southbound on Robinson Road between Greenhurst Road and Locust Lane that passed him shortly before the crash. The Dodge truck may have some damage to the front end from a previous crash.

Troopers would like to talk with the driver of this truck as a witness to the crash. The truck was not involved in the crash or a cause of the crash.

On Wednesday , August 18, 2015, at 8:56 a.m., the Idaho State Police investigated a single vehicle,injury crash northbound on SH28 at milepost 52, near Lone Pine. Paul Bennett, 34, of Lewisville, was driving a 2002 Sterling 7500 truck northbound. Bennett went off the shoulder, overcorrected and coming to rest on the left shoulder.


Bennett was air lifted to Eastern Idaho Regional Medical Center. Bennett was wearing his seatbelt at the time of the crash. The Idaho State Police was assisted by the Clark County Sheriff and Lemhi County Sheriff.

On Wednesday, August 19, 2015, at approximately 6:10 a.m., the Idaho State Police investigated a two-vehicle, fatality crash at the intersection of Robinson Road and Locust Lane, east of Nampa.

Sharalyn Rose, 28, of Meridian, was eastbound on Locust Lane in a 2001 Chevrolet Lumina. Paul Mackrill, 44, of Nampa, was southbound on Robinson Road in a 2009 Yamaha WR450F motorcycle. On August 17, 2015, at approximately 11:49 A.M., Idaho State Police investigated a two-vehicle injury crash on eastbound Interstate 84 at milepost 51, in Boise.

Frank Cordoba, 58, of Aloha, Oregon, was stationary on the right shoulder in a 2016 Freightliner Tractor. Alejandro Juarez, 19, of Meridian, was traveling eastbound in a 2014 Nissan Quest van. Juarez drove off the right shoulder and struck the rear of the 2016 Freightliner. Both Juarez and Cordoba were transported by ground ambulance to St. Alphonsus Regional Medical Center in Boise.

Cordoba was wearing his seatbelt. It is unknown if Juarez was wearing a seatbelt. The two right lanes were blocked for approximately a hour and a half. This crash is under investigation by Idaho State Police.

On Sunday, August 16, 2015, at 3:52 a.m., the Idaho State Police investigated a vehicle versus pedestrian fatality crash on I-84 at milepost 68, east of Boise. Winters Wareagle, 23, of Garden City, and his passenger, Melanie Crutcher, 42, of Garden City, were traveling eastbound in a 2003 Kia Spectra when they started to have mechanical problems, and pulled onto the shoulder of the road. Wareagle exited the vehicle and stepped out into traffic. Patrick Musgrove, 28 of Meridian, was traveling eastbound in a 2003 Toyota Matrix. The Toyota struck Wareagle. Wareagle succumbed to his injuries on scene. Musgrove was arrested and charged with Aggravated DUI.

Notifications have been made to the family have been made. Both eastbound lanes were blocked for approximately 3 hours. This crash is under investigation by the Idaho State Police.

On Sunday, August 16, 2015, at 6:22 a.m., the Idaho State Police investigated a one-vehicle rollover in Caribou County.

Gavan Smith, age 21, of Soda Springs, was driving southbound on Government Dam Road in a 2007 Pontiac G6 approaching the intersection for China Cap Road. Approximately two miles north of the intersection, Smith lost control and the vehicle rolled. Smith and his two passengers were not wearing seatbelts; the passengers were ejected and unknown if Smith was ejected at this time.

The passengers, Tyler Fetzer, age 20, and Damian Henderson, age 21, both of Soda Springs, were taken by ground ambulance to Caribou Memorial Hospital in Soda Springs. Both were later transported by air ambulance to Portneuf Medical Center in Pocatello. Smith was arrested for Aggravated DUI.

On Saturday, August 15, 2015 at approximately 10:23 a.m., the Idaho State Police investigated a two-vehicle, injury crash on US-91 at milepost 83.5, north of Chubbuck.

Elizabeth Loynd, 27, of Shelley was northbound on US-91 in a 2010 Ford Ranger. Marcia Chapo, 21, of Fort Hall was also northbound in a 2007 Toyota Camry. Loynd slowed to perform a U-turn. While Loynd was executing the U-turn, the Camry struck the Ranger on the driver's side.

Both Chapo and Loynd were transported to Portneuf Regional Medical Center by ground ambulance. Loynd was wearing her seatbelt; Chapo was not. The northbound lanes were blocked for approximately twenty minutes while crews worked to clear the scene.

The crash is under investigation by the Idaho State Police.

On August 14, 2015, at approximately 3:50 P.M., the Idaho State Police investigated a single vehicle crash on US Highway 93 at milepost 324.

Carl Kiser, 42, of Jerome, was driving a 2007 Chevrolet Silverado pickup northbound when he drove off the right shoulder, overcorrected, and then drove off the left shoulder. The vehicle overturned, and struck a power pole, coming to rest on its top.

Kiser was not wearing a seatbelt, was ejected, and succumbed to his injuries at the scene.

Next of kin has been notified.

The crash is under investigation by the Idaho State Police.

On Friday, August 14, 2015, at approximately 2:38 p.m., the Idaho State Police investigated a five-vehicle, property damage crash westbound I-84 just before the 44 off-ramp.

David Foruria, 54, of Emmett, was westbound on I-84 in a 1987 Ford Bronco. Leah Clements, 55, of Fruitland, was ahead of the Bronco in a 2008 Toyota Camry. Kholbek Boynazarov, 19, of Meridian, was ahead of the Camry in a 2010 Dodge Charger. Sharon Anitok, 54, of Boise, was ahead of the Charger in a 2009 Subaru Forester.

When traffic ahead slowed, Foruria braked, but the Bronco struck the rear of the Camry. The Camry then struck the Charger, which struck the Subaru. Mallory Dardis, 19, of Nampa, was behind the Bronco in a 1998 Ford Taurus. After the Bronco struck the Camry, the Taurus struck the rear of the Bronco.

All occupants were wearing their seatbelts. The right lane was blocked for approximately one hour.

The crash is under investigation by the Idaho State Police.

On Friday, August 14, 2015, at approximately 10:14 a.m., the Idaho State Police investigated a four-vehicle, injury crash westbound I-184 at milepost 3.5, west of the Fairview off-ramp.

Horace Hunt, 58, of Boise, was westbound on I-184 at milepost 3.5 in a 2004 Volvo XC70 station wagon in lane two when he struck a deer. Hunt drove to the median shoulder, but was partially blocking lane one. Cindy Nielsen, 61, of Boise was in lane two in a 2011 Ford Econoline van. Christine Pietz, 45, of Flagstaff, AZ, was behind the Econoline van in a 2004 Honda Odyssey. The Econoline van slowed down because of the crash. Pietz braked and steered to the left, entering lane one. Benjamin Haws, 15, of Meridian, was westbound in lane one in a 1993 Ford pickup. The Ford pickup struck the back of the Honda. The Honda then struck the driver's side of the Econoline van, and the rear of the Volvo XC70.

All occupants were wearing their seatbelts. Pietz and a juvenile passenger in her vehicle were both transported to St. Alphonsus Regional Medical Center in Boise by ground ambulance. Lanes one and two were blocked for approximately one-and-a-half hours.

The crash is under investigation by the Idaho State Police.

On August 12, 2015, at approximately 5:11pm, the Idaho State Police conducted a traffic stop on Sunnyside Road near Interstate 15. The driver, Cesar G. Herrera-Sanchez, 37 , Billings Montana, was arrested for driving under the influence of drugs. The trooper located inside Herrera-Sanchez's vehicle approximately 4.67 ounces of methamphetamine and approximately 18.10 grams of heroin. The approximate value of the drugs is $7,000.00. The passenger of the vehicle, Christopher L. Haynes, 29, Billings Montana, was also arrested due to the drugs in the vehicle. Both Herrera-Sanchez and Haynes, were booked into the Bonneville County Jail for Trafficking Heroin and Methamphetamine as well as possession of drug paraphernalia. Herrera-Sanchez was also charged with Driving Without Privileges and Driving Under the Influence.

INCIDENT NARRATIVE: Thomas was traveling southbound US 95 near milepost 461.4 when she veered into the northbound lane striking the drivers side of the Semi driven by Muntz. Thomas vehicle was launched off the west shoulder of the highway and over turned. The scene covered a quarter mile of roadway and the highway was blocked for 4 and a half hours. The crash is still under investigation by the Idaho State Police.

On Wednesday, August 12, 2015, at approximately 6:12 P.M., the Idaho State Police investigated a four vehicle crash on Interstate 84, near mile post 46, near the Eagle off ramp.

Samson L. Bear, 36, of Fort Hall, ID, was driving west bound on Interstate 84 in a 2007 Dodge R2500. Spencer L. Washakie, 26, of Pocatello, ID, was driving behind Bear in a 2003 Chevrolet Silverado. Anthony T. First, 41, of Pocatello, ID,was driving behind Washakie in a 1997 Ford F350. Johnny P. Shay, 26, of Pocatello, ID, was driving behind First in a 1997 Ford Expedition.

Shay failed to slow for traffic, and collided with First, who then collided with Washakie, who then collided with Bear.

Shay and his two occupants were transported by ground ambulance to Saint Lukes Meridian Medical Center. It is unknown if any of the occupants were wearing seatbelts. The left lane was blocked for approximately one hour and fifteen minutes.

On Wednesday, August 12, 2015, at approximately 10:52 P.M, Idaho State Police investigated a single vehicle crash westbound Interstate 84, near mile marker 72.

James Lubacky, 54, of Star, was driving westbound on Interstate 84, near mile marker 72, in a 1992 Jeep Wrangler. Lubacky drove off the right shoulder and contacted the rumble strips. He then overcorrected, drove through the median, and across the eastbound travel lanes before overturning the Jeep on the right shoulder of the eastbound lanes. Lubacky was not wearing a seatbelt, and was ejected from the Jeep. He was transported by air ambulance to Saint Alphonsus Regional Medical Center, in Boise.

The eastbound lanes were blocked for approximately one hour. This crash remains under investigation by the Idaho State Police.

On Wednesday, August 12, 2015 at approximately 10:21 a.m., the Idaho State Police investigated a single-vehicle, injury crash westbound I-184 at milepost 0.

Betty Privett, 40, of Kuna, was westbound on I-184 in the yellow 2 section in a 1997 Isuzu Rodeo. The Rodeo went off the right shoulder, overcorrected toward the left and then to the right, after which the vehicle left the roadway on the left shoulder. The Rodeo slid sideways down a gravel embankment and through the median before overturning. The vehicle came to rest in the left lane of the yellow 3 section.

Privett was wearing her seatbelt. She was transported to St. Alphonsus Regional Medical Center in Boise by ground ambulance. Both lanes of the yellow 3 section were blocked for approximately one hour.

This crash is under investigation by the Idaho State Police.

On August 11, 2015, at approximately 4:34 p.m., Idaho State Police investigated a two-vehicle crash on State Highway 16, near Floating Feather Road, just north of Star. Alan Jones, 54, of Caldwell, was driving northbound on Highway 16 in 2015 Chevrolet Silverado pickup. Kristina Gonzales, 37, of Oroville, California, had pulled over on the side of the road in a 2002 Dodge Dakota pickup. Gonzales turned left from the right shoulder of northbound State Highway 16 in an attempt to turn around. As Gonzalez was turning, her vehicle was struck on the driver's side by the passenger side of Jones' vehicle.

Jones was transported via ground ambulance to St. Alphonsus Regional Medical Center in Eagle. Both drivers were wearing seat belts.

This crash is under investigation by the Idaho State Police.

On Tuesday, August 11, 2015, at approximately 3:50 P.M., the Idaho State Police investigated a two vehicle crash on State Highway 55 near mile post 81.5, two miles north of Banks, ID.

Michael J. Stwan, 49, of Boise, ID, was driving northbound on State Highway 55 in a 2003 Kia Sorento. Stwan crossed the center line and collided with Eric P. Larson, 54, of Boise, ID, who was driving southbound in a 2012 Buick Regal. The Kia Sorento overturned and came to rest on its top and caught fire. Several logging truck drivers stopped to assist, taking fire extinguishers out of their vehicles to put out the fire.

Stwan and Larson were both transported to Saint Alphonsus in Boise, ID, by air ambulance. Both drivers where wearing seat belts. The roadway was blocked for approximately three hours.

On August 11, 2015 at approximately 12:46 a.m., Idaho State Police investigated two separate crashes northbound Interstate 15 at milepost 75.3, north of Pocatello. Tyler Arnold, age 28, of Salt Lake City, Utah was driving southbound in a 2013 Freightliner commercial vehicle in the left lane. Arnold left the roadway on the left shoulder and struck the guardrail on the east side of the southbound lanes. His vehicle traveled through the median and impacted the guardrail on the west side of the northbound lanes of Interstate 15 and came to rest.

Austin Shippen, age 21, of Menan, Idaho, was driving northbound on Interstate 15 in a 1996 Honda Civic. Chelsea Barnes, age 21, of Blackfoot, Idaho, was also driving northbound on Interstate 15 in a 1994 Nissan Altima. Shippen's vehicle slowed and was rear ended by Barnes' vehicle.

The northbound left lane of Interstate 15 is still blocked at this time. The drivers of all three vehicles were wearing their seatbelts. The crashes are under investigation by Idaho State Police.

On August 9, 2015, at approximately 5:22 a.m., Idaho State Police investigated a single-vehicle injury crash at eastbound County Highway 17 at milepost 1, near Banks. Kylus Hart, 23, of Avon, Colorado, was traveling westbound on County Highway 17 in a 2004 Ford Pickup. Hart fell asleep, failed to negotiate a curve, and drifted off the south edge of the roadway. The vehicle rolled down an embankment and came to rest partially in the south fork of the Payette River. Hart was extricated from the vehicle and was transported by air ambulance to St. Alphonsus Regional Medical Center in Boise. Hart's passenger, Jessica Monis Hernandez, 21, of Horseshoe Bend, was transported by private vehicle to Chevron in Horseshoe Bend before being transported by air ambulance to St. Alphonsus. It is unknown if either occupant was wearing a seatbelt. This crash is under investigation by Idaho State Police.

On August 8, 2015, at approximately 4:27 p.m., Idaho State Police investigated a single vehicle rollover crash eastbound on Interstate 84, at milepost 254, near Malta. John Bisengimana, age 35, of Boise, was driving eastbound in a 2005 Nissan Xterra. Bisengimana drove off the left side of the roadway, overcorrected, and came back onto the roadway. The vehicle left the roadway once more before it rolled. The vehicle came to rest facing westbound in the eastbound lanes of travel of the Interstate.

A passenger in the vehicle, Rose Uwimana, age 47, of Boise, was not wearing her seatbelt and was ejected. Uwimana succumbed to her injuries at the scene of the crash. Notification to Uwimana's family has been made. A second passenger, Kabura Meliki, age 62 of Boise, also succumbed to his injuries at the scene of the crash. Notification to Meliki's family has been made. Bisengimana and a third passenger, Nsabimana Victoria, age 49 of Boise, were transported by air ambulance to Portneuf Medical Center in Pocatello.

Both eastbound lanes of travel were blocked for approximately 6 and a half hours while crews worked to clear the scene. The crash is under investigation by Idaho State Police.

On Saturday, August 08, 2015, at 2:49 p.m., the Idaho State Police investigated a single-vehicle injury crash on Interstate 15 near McCammon.
Jack Gilbert Jr, 59, of Rio Rancho, New Mexico, was driving a 2010 Victory motorcycle. Gilbert was southbound on Interstate 15, taking the exit for McCammon at milepost 47 when he hydroplaned and crashed on the off-ramp. He was wearing a helmet and was transported by ground ambulance to Portneuf Medical Center in Pocatello.

This crash is under investigation by the Idaho State Police.

On Friday, August 7th, 2015, at approximately 8:33 a.m., the Idaho State Police investigated a single vehicle crash on Lower Pleasant Ridge Road east of US95 near Wilder. Jorge Arredondo, 19, of Wilder, was traveling westbound in a 2007 Nissan Sentra, when he lost control of the vehicle, left the roadway, and rolled multiple times. Arredondo was ejected from the vehicle.

Arredondo was transported by ground ambulance to Saint Alphonsus in Boise. He was not wearing a seatbelt. This crash remains under investigation by the Idaho State Police.

On August 7, 2015, at approximately 11:25 a.m., the Idaho State Police investigated a single-vehicle, rollover crash on Interstate 86 at milepost 8, east of Burley.

Jenna Womack, 18, of Pocatello, was driving eastbound in a 2003 Saturn sedan. Womack had passed a semi when she lost control of the vehicle and then overcorrected. The vehicle went off the shoulder, rolled, and came to rest on its wheels in the median.

Womack's passenger, Micah Cram, 19, of Gooding, was transported via air ambulance to Portneuf Regional Medical Center in Pocatello. It is unknown at this time if he was wearing a seatbelt. Womack was transported via ground ambulance to Cassia Regional Medical Center in Burley. Womack was wearing a seatbelt at the time of the crash.

The left lane was blocked for emergency vehicles for approximately an hour-and-a half while crews worked to clear the scene.

The crash is under investigation by the Idaho State Police.

On August 6, 2015, at approximately 6:35 p.m., Idaho State Police investigated a single vehicle injury crash on State Highway 33, near milepost 63, in Jefferson County.

Hannah E. Burlingham, 22, of Hamilton, MT, was driving eastbound on State Highway 33 in a 2008 Hyundai Accent. She drove off the right side of the road, overcorrected, and lost control of the vehicle. She again drove off the right side of the road and down an embankment, where she crashed into a pile of rocks.

Burlingham was transported by ground ambulance to Madison Memorial Hospital in Rexburg, ID.

Idaho State Police was assisted by Jefferson County Sheriff's Office.

The investigation is ongoing at this time.

On August 6, 2015, at approximately 3:20 p.m., Idaho State Police investigated an injury crash on State Highway 27, near milepost 13.9, in Burley, ID.

A semi truck loaded with straw was on fire on the west side of the road. Fire crews were on scene, battling the fire. Shaylee Alexander, 29, of Burley, ID., was driving southbound on State Highway 27 in a 2014 Chevrolet pickup. Alexander's vision was obscured by the smoke as she drove through the area, and she drove into a fire truck.

Alexendar was wearing a seatbelt at the time of the crash. She was transported by ground ambulance to Cassia Regional Medical Center in Burley, ID.

Below is the information received from ODOT PIO, Thomas Strandberg, in reference to this re-opening of I-84:

"I-84 Eastbound and Westbound freeways are now open in eastern Oregon between Pendleton (Exit 216) and Ontario (Exit 371). Travelers are reminded to drive with extra caution, don't speed and watch for crews near fire scene. Road closures are frustrating, but let's all get home safe. OR 245 south of Baker City, and OR 203 and 237 between La Grande and North Power are also now open for travel. OR 204 (Tollgate Hwy) open to local traffic, but due to the Phillips Creek Fire burning north of Elgin, through traffic motorist are advise to use alternate routes. On Wednesday, August 05, 2015, at approximately 12:49 p.m., the Idaho State Police investigated a three vehicle crash eastbound I-84 near the Locust Grove overpass, in Meridian.

Maria L. Anderson, 34 of Payette, ID, was driving a 2009 Toyota Matrix eastbound on I-84 near the Locust Grove overpass. Anderson made an unsafe lane change side swiped a 2009 Kenworth semi pulling a single trailer, driven by Michael H. Thomas, 51 of Boise, who was also traveling eastbound. Anderson then drove back into the lane and was struck on the driver side door by Catherine I. Ledesma, 51 of Whittier, CA, driving a 2015 Chevrolet Impala.

All drivers where wearing their seatbelts. The left lane, currently the bi-pass lane in the construction zone, was blocked for approximately one hour and fifteen minutes.
