INCIDENT NARRATIVE: Demean was North bound on US95 near milepost 338 when he failed to maintain his lane, he overcorrected and lost control of the Ford. Demean then left the roadway and rolled the vehicle. The highway was blocked North bound for 2 hours.

Idaho Operation Lifesaver "Officer on a Train"

Officer On A Train and Adopt A Crossing

On Tuesday October 2nd, City, County and State Law Enforcement Officers will be conducting an Officer On A Train and Adopt A Crossing enforcement operation in North Idaho.

The purpose of this enforcement operation is to increase public awareness of the potential dangers that exist at highway railroad intersections and eliminate driver actions that can have tragic consequences, by enforcing the traffic laws that pertain to these intersections. The enforcement program is one of three parts of the Operation Lifesaver effort, which include engineering and education, designed to eliminate car train collisions.

During the Officer On A Train enforcement effort, one officer is placed in the lead locomotive of a train as a spotter. This officer observes traffic approaching the highway rail intersections as the train proceeds down the tracks. Other officers pace the train or are parked at specific locations. When a motorist is observed violating the laws pertaining to the approaching intersection, the officer on the train radios one of the chase cars which stop the motorist, explain the dangers and issue a citation.

With the Adopt A Crossing approach, City, County and State Law Enforcement officers from throughout Southeast Idaho will pick a crossing within their jurisdiction and enforce the laws that pertain to the crossing.

During 2011, there were 15 car train collisions resulting in 3 people injured and 2 fatalities. This year to date, there have been 6 car train collisions reported, resulting in 2 people injured and 1 fatality.

Since 1990, the Officer On A Train and Adopt A Crossing programs have been instrumental in helping reduce the number of car train collisions in the state. City, County and State law enforcement, State and federal agencies, along with the private business and the railroads that operate in the state sponsor the Idaho Operation Lifesaver program.

On Saturday, September 29, 2012, Idaho State Police investigated an injury motorcycle crash on State Highway 36 near Weston, Idaho.

Curtis B. Hardy, age 54 of Grace, was riding his 2008 Harley Davidson motorcycle east on State Highway 36 near mile post 119. Hardy lost control of his motorcycle and went off the road. Hardy was not wearing a helmet and was transported by ambulance to the Franklin County Medical Center. This crash is still under investigation by the Idaho State Police.

On September 27, 2012, Idaho State Police Detectives and Gooding County Sheriff's Office Deputies responded to a tip of a large amount of marijuana growing in a corn field near 3300 South, in Gooding County. Law enforcement located 3 marijuana grow sites, which contained approximately 2,649 plants, with an estimated street value of 5 million dollars. The investigation is ongoing at this time. ISP and Gooding County are asking if you have any information regarding these grow sites or others like it, to report it to the ISP District Four Office or to the Gooding County Sheriff's Office.

On September 27, 2012, at approximately 1:15 p.m., Benjamin P. Tveidt, 26, of Boise, was traveling south on I-15 near milepost 119 operating a military hummvee pulling a utility trailer. Tveidt was following the vehicle in front of him too closely, and was unable to stop when traffic slowed for the construction zone. Tveidt locked up his brakes and swerved, causing his trailer to overturn.

ISP was assisted by ITD and sign crews from the construction zone.

On September 24, 2012, at approximately 5:02 p.m., Idaho State Police investigated a one vehicle roll over crash on Interstate 15 near milepost 105, south of Shelley, Idaho.

David Meeker, 50, of Blackfoot, was driving a 1985 Ford Bronco II north on Interstate 15 near milepost 105 when a tire blew. Meeker lost control and the vehicle rolled into a field just off Interstate 15. Meeker was transported by ground ambulance to Eastern Idaho Regional Medical Center in Idaho Falls. Meeker was not wearing a seat belt.

On Saturday, September 22, 2012, at approximately 3:54 p.m., Idaho State Police investigated a two- vehicle injury crash on U.S. highway 20 outside of Ashton, Idaho.

Judith A. Barth, age 59 of Lapine, Oregon, was traveling east on US20 in a 2000 GMC Yukon pulling an oversized camp trailer. Barth was attempting to slow down for Ashton city limits when she lost control of the trailer at approximately mile post 360. The trailer fish-tailed and side swiped a 2010 Jeep that was headed west. The trailer proceeded to roll causing complete blockage of US20 for about 2 hours.

The driver of the Jeep, Donald E. Echelmeyer, age 50, and his passenger Katherine R. Wiseheart , age 49, both of Jackson, Wyoming were transported by ambulance to Madison Memorial Hospital in Rexburg. No injuries were reported in Barth's vehicle. All occupants were wearing seatbelts.

Colton Olson, 28, of Caldwell, was driving east on the 44 on ramp in a commercial Freightliner when he lost control and it overturned. The Freightliner is still currently blocking the on ramp. Crews are working to remove the vehicle.

On September 20, 2012, at approximately 7:15 p.m., the Idaho State Police investigated a two vehicle fatality crash on US26 at milepost 361, approximately 12 miles east of Ririe.

Shaine White, 37, of Meridian, was driving his 1993 Honda Accord west on US26. Richard Hotchkiss, 56, of Twin Falls, was driving a 2008 Kenworth commercial vehicle east on US26. White crossed the center line entering the eastbound lane. Hotchkiss attempted to avoid White by steering to the right, but White's vehicle struck the commercial vehicle, causing the commercial vehicle to catch fire.

Burns D. Leavitt, 45, of Pocatello, was driving behind White on his 2012 Harley Davidson motorcycle. Leavitt and his passenger Burns M. Leavitt, 19, of Pocatello were struck by debris from the initial crash but were not injured.

White was not wearing a seatbelt and died at the scene. Hotchkiss was wearing a seatbelt and transported by ground ambulance to Eastern Idaho Regional Medical Center in Idaho Falls.

This crash is under investigation by the Idaho State Police.

The next of kin of the passenger has been notified. The passenger was Hugo Silva-Avalos, 26, of Idaho Falls.

On Sunday, September 16, 2012, at approximately 9:24 pm, Idaho State Police investigated a one vehicle fatality crash on Interstate 84 near Bliss.

Oscar Hernandez-Ramirez, 28, of Caldwell, was driving west on Interstate 84 in a 1994 Honda Accord. Oscar drove off the road way and overcorrected causing the vehicle to roll. On Sunday, September 16, 2012, at approximately 9:24 pm, Idaho State Police investigated a one vehicle fatality crash on Interstate 84 near Bliss.

Oscar Hernandez-Ramirez, 28, of Caldwell, was driving west on Interstate 84 in a 1994 Honda Accord. Oscar drove off the road way and overcorrected causing the vehicle to roll. Oscar was ejected from the vehicle and succumbed to his injuries at the scene of the crash. His passenger died at scene as well. The passenger was wearing a seatbelt.

Interstate 84 was blocked for approximately 3 hours.

The passenger's name will be released at a later time.

On September 16, 2012, at approximately 7:51 p.m., Idaho State Police investigated a one vehicle roll over fatality-injury crash on State Highway 37 at approximately milepost 65, near Indian Springs, in Power County, Idaho.

La Khrista Rigdon, 27, of American Falls, was driving her 2008 Pontiac G6 south on State Highway 37. The vehicle veered off the roadway, Rigdon over corrected and the vehicle rolled several times. Jose Tores JR, 25, of American Falls, a passenger and Rigdon died at the scene. Three children were transported to Harms Memorial Hospital in American Falls. Two of the children were later taken by air ambulance to Portneuf Medical Center in Pocatello.

The crash is under investigation by Idaho State Police.

On September 16, 2012, at approximately 12:25a.m., Idaho State Police investigated a two vehicle crash on US91 near mile post 85 in Bannock County near Chubbuck. Danny Bailey, 31 of Blackfoot, was traveling northbound, Bailey pulled over and attempted to make a u-turn on US91 in a 1995 Cadillac Deville. Juana Rembao, 41 of Chubbuck, was traveling southbound on US91 in a 1996 Chevy truck and collided with Bailey.
Rembao was transport by ground ambulance to Portneuf Medical Center in Pocatello. The occupants of the Chevy were wearing their seatbelts. Rembao and his passenger, Heather Martinez, 35 of Fort Hall were not wearing their seatbelts.
The road was partially blocked for approximately 1 hour for the investigation of the crash.

On Friday, September 14, 2012, at approximately 12:35 pm, Idaho State Police investigated a two-vehicle, injury crash on Interstate 15 near Pocatello.

Christina Kolodziej, 43 of Emmett, Idaho, was driving a 2006 Ford Escape north on Interstate 15 near the junction with Interstate 86. When Kolodziej missed the exit for Interstate 86, she turned left to turn around on the Interstate. Kolodziej's vehicle was struck in the side by 2004 Nissan Altima driven by Robert Grannis, 68 of Sisters, Oregon.

Kolodziej, Grannis, and Grannis' passenger, Betty Grannis, 67 of Sisters, Oregon, were transported to Portneuf Medical Center in Pocatello. All involved were wearing seatbelts.

On Saturday, November 23, 1935, Mildred Hook was kidnapped off the streets in Tacoma, Washington, by Douglas F. Van Vlack. Police throughout the Northwest were alerted to be on the lookout for the couple. At 2:00 P.M. on Monday, November 25, 1935, Idaho State Trooper Fontaine Cooper and Twin Falls Deputy Sheriff Henry C. Givins spotted Van Vlack's 1931 Ford coupe on Highway 30 near Buhl. The officers pulled Van Vlack over. As Trooper Cooper approached the car, Van Vlack pulled out a pistol and shoot Trooper Cooper, killing him instantly. Van Vlack then shot Deputy Givens three times; Deputy Givens died two weeks later. Trooper Fontaine Cooper was the first trooper killed in the line of duty. On Friday, September 14, 2012, at 1100 A.M., the Idaho State Police will dedicate the District Four Office in the memory of Fontaine Cooper. Current and retired officers, citizens, elected officials and the media are encouraged to attend this historic dedication. The address is 218 West Yakima in Jerome.

On September 11, 2012 at approximately 7:02 pm, Idaho State Police investigated a single commercial vehicle property damage crash on State Highway 33 near milepost 15. Dexter Hill, 28, of Pocatello, ID, was traveling south on State Highway 33 in a red 2010 Kenworth semi hauling two trailers full of grain. Hill drove off the right side of the road and hit a culvert overturning his vehicle and detaching the rear most trailer. South State Highway 33 was fully blocked for approximately 7.5 hours and the north lanes were partially blocked for 4 hours.

Dexter was wearing a seatbelt.

The crash remains under investigation. Idaho State Police were assisted by Butte County Sheriff's Office and ITD.

On Tuesday, September 11, 2012, at 7:38 pm, Idaho State Police investigated a vehicle versus pedestrian injury crash near the intersection of Main Street and Idaho Street, in Wendell. Jesseca Dodge, 24, of Wendell, was driving west on Main Street in a Dodge Caravan. Dodge was in the left lane of travel. Roney Welker, 36, of Wendell, was crossing Main Street, approximately 200 feet east of the marked cross walk. Dodge's vehicle struck Welker in the middle of the street. Welker was transported via air ambulance to St. Alphonsus Regional Medical Center in Boise.

The setting sun was a factor in this crash. Welker had consumed alcohol prior to the crash. All occupants in the vehicle were wearing seat belts.

In July 2011, the Idaho Office of Highway Safety (OHS) and the Idaho State Police (ISP) entered into a partnership which created a full time position of the State Impaired Driving Coordinator (SIDC). The SIDC position was created to provide oversight to three of Idaho's essential impaired driving programs. The SIDC is responsible for management of the Drug Evaluation and Classification Program (DECP), Standardized Field Sobriety Testing program (SFST) and Law Enforcement Phlebotomy Program (LEPP). Sgt. Dean Matlock of the Idaho State Police assumed the position of SIDC on July 1, 2011, splitting his duties between the Idaho State Police Training Section and the SIDC position until a new training section sergeant was selected. On October 1, 2011, Sgt. Matlock moved into the SIDC position in a full time capacity.

As part of the continuing partnership between OHS and ISP, OHS purchased a new breath testing instrument that is to be used to train Idaho Law Enforcement. On August 2, 2012, ISP took possession of a new Intoxilyzer 5000. The new Intoxilyzer will be used for training new officers, recertifying current officers and other uses as needed.

Due to the partnership of ISP and OHS, new officers will be better prepared in the detection and apprehension of alcohol/drug impaired drivers and able to administer the breath alcohol test(s) required by lawmaking Idaho's roadways safer.

On Tuesday, September 11, 2012 at 3:35 am, the Idaho State Police investigated a one vehicle rollover on the westbound 46 off ramp of Interstate 84. Usiel Rios, 23, of Boise, was driving a 2001 blue Ford Focus westbound on I84 when he took the 46 (Eagle) off ramp, lost control of the vehicle and rolled off the right shoulder. Rios' wife, Araceli Brito-Cardoso, 34, of Boise and their one year old daughter, Maria Rios-Cardoso, were in the back seat unrestrained and were ejected from the vehicle.

All three occupants were transported to St. Alphonsus Regional Medical Center with non-life threatening injuries. Usiel and Maria were treated and released from the hospital; while Araceli remains in the hospital with moderate injuries. Araceli was not wearing a seatbelt and Maria was not in a child safety restraint. Alcohol may be involved in the crash.

The Idaho State Police were assisted at the scene by Meridian Police Department and Ada County Fire and Paramedics. The crash is still under investigation.

During the week of September 9-15, 2012, the Idaho State Police Commercial Vehicle Safety Section will be participating with other safety agencies throughout the country in an event called Brake Safety Week. Brake Safety Week is focused on enhancing knowledge, regulatory compliance and performance of large truck braking systems.

Certified Inspectors, brake suppliers, and industry partners conduct enforcement and education activities with drivers and mechanics at weigh stations and other locations as a part of the Operation Air Brake Campaign. Since large trucks weigh much more than passenger vehicles, it takes them a longer distance to stop. Improperly adjusted or worn out brakes can dangerously increase truck stopping distances. Just like keeping your own vehicle operating safely is important, so is careful preventative maintenance on large trucks and buses.

The goal of the Idaho State Police is to reduce the number of crashes that occur on Idaho's roads. Brake Safety Week is an opportunity proactively to reduce the risks that contribute to crashes through enforcement, education and outreach to drivers, mechanics and companies on the importance of proper brake inspection, maintenance and operation. We want everyone to arrive at their destination safely. Let's work together to keep our roadways safe.

On September 9, 2012, at approximately 8:00 a.m., Idaho State Police investigated a single-vehicle injury crash on State Highway 48 near milepost 20, between Rigby and Ririe. Jessica Jones, 22, of Ririe, was eastbound in a 2008 Honda Accord. Jones failed to negotiate the curve, and the vehicle went off the left shoulder, striking several trees.
Airbags were deployed, and Jones was transported by ground ambulance to Eastern Idaho Regional Medical Center in Idaho Falls. Alcohol was involved.
The crash is under investigation.

On September 8, 2012, at approximately 3:20 p.m., Idaho State Police attempted to stop an orange 2009 KTM motorcycle on Interstate 84 near milepost 165, in Jerome County. The motorcycle was driven by Nicholas Jensen, 27 of Columbus, Indiana. Jensen was traveling east at 90 miles per hour in a posted 75 mile per hour speed zone, when he failed to yield for a traffic stop. Jensen took the Jerome exit and continued to drive away at a high rate of speed. Jensen drove into a near-by field, where Jerome County, Jerome Police Department and the Idaho State Police set up a perimeter. Jensen was found in the yard of a near-by residence and was taken into custody.

On Saturday, September 8th, 2012 at 9:11 am, the Idaho State Police investigated a 2 vehicle fatality crash on Old Highway 30 at Montgomery Road near King Hill. Vincent Porter, 96 years old, from King Hill, was driving a 2005 Toyota Tacoma northbound on Montgomery road. Maria Salazar, 46 years old, from King Hill, was traveling southbound on Montgomery road in a 2003 Chevy Trailblazer. Porter turned eastbound onto Old US30 and struck the back of the Trailblazer. The Trailblazer slid sideways and overturned, ejecting both Salazar and her 9 year old daughter. Salazar succumbed to her injuries at the scene of the crash. Salazar's 9 year old daughter was transported to Saint Alphonsus Regional Medical Center in Boise, and was later transported to Saint Lukes in Boise. Porter was transported by ground ambulance to Elmore Medical Center. Salazar and her daughter were not wearing seatbelts. Porter was wearing his seatbelt.

This crash in under investigation by the Idaho State Police.

The driver of the 2012 Ford Focus, Raymond Kunz, 61, of Colorado Springs, the the driver of the 1998 Mazda Protege, Kamal Darji, 22, and Sha Darji, 64, a passenger in the Mazda, were transported to North Canyon Medical Center in Gooding, Idaho. Khadga Darji, 19, a passenger of the Mazda was flown by life flight to St. Alphonsus Regional Medical Center in Boise, Idaho. Nar Darji, 20, and Harka Darji, 55, both passengers of the Mazda, succumbed to their injuries at the scene of the crash.

On Thursday, August 30, 2012, at approximately 3:46 p.m., Idaho State Police investigated a two vehicle fatality crash on Interstate 84 near mile post 134.5 in Gooding County.

A maroon Mazda Protege was on the median side and partially in the lane of travel. A red Ford Focus was traveling east when it rear ended the stationary vehicle causing the Protege to go down into the median and the Ford to roll and land on its side.

Multiple subjects were transported by air and ground ambulance. Two subjects succumbed to their injuries at the scene of the crash.

Interstate 84 was blocked on the east bound side for approximately two and a half hours.

This crash is under investigation and the names of the occupants will be released when notifications have been made.

The Idaho State Police is currently investigating an injury crash eastbound I84 at milepost 134.5, near the Bliss Rest Area, involving several vehicles. All eastbound lanes are shut down at this time and motorists are encouraged to avoid the area. More information to follow as it becomes available.

On September 2, 2012, at approximately 11:45 p.m., Idaho State Police investigated a fatal-injury crash at 620 N Menan-Lorenzo Hwy, Menan, Jefferson County, ID.

Timothy A. Merrill, 19, of Rigby, ID, was driving a 1992 GMC pickup truck east on the Menan-Lorenzo Hwy. Merrill failed to negotiate a curve at 620 N, and drove his vehicle off the right shoulder. Merrill lost control of the vehicle and the vehicle overturned.

Passengers in the vehicle were Keely M. Lance, 18, of Rigby, ID; Matthew P. Walker, 19, of Menan, ID; Trevor A. Davis, 19, of Rigby, ID; Madison A. Holm, 17, of Roberts, ID; and Samantha M. Wood, 16, of Roberts, ID. None of the occupants in the truck were wearing seatbelts. Merrill and all five passengers were ejected from the vehicle.

Keely M. Lance was pronounced dead at the scene. Trevor A. Davis was transported by air ambulance to Eastern Idaho Regional Medical Center in Idaho Falls, ID. Timothy A. Merrill, Matthew P. Walker, Madison A. Holm, and Samantha M. Wood were transported by ground ambulance to Eastern Idaho Regional Medical Center in Idaho Falls, ID.

The crash investigation is continuing.

** Original Release **


On September 1, 2012, at approximately 5:15 p.m., a single vehicle crash occurred on the westbound lanes of Interstate 84 near milepost 225, east of Burley. A 2007 Honda Accord, driven by Rosemarie Edvalson, 56 of Eagle, was traveling westbound, when she changed lanes after passing another vehicle. Edvalson's vehicle then hydroplaned in a large puddle that formed in the lanes of travel due to severe weather that was occurring in the area. Edvalson's vehicle flipped and landed upside-down off of the roadway in a large amount of standing water. On September 1, 2012, at approximately 5:15 p.m., a single vehicle crash occurred on the westbound lanes of Interstate 84 near milepost 225, east of Burley. A 2007 Honda Accord, driven by Rosemarie Edvalson, 56 of Eagle, was traveling westbound, when she changed lanes after passing another vehicle. Edvalson's vehicle then hydroplaned in a large puddle that formed in the lanes of travel due to severe weather that was occurring in the area. Edvalson's vehicle flipped and landed upside-down off of the roadway in a large amount of standing water. Edvalson and her passenger, Juanita Spitzer, 75 of Nampa, were extricated and transported to Cassia Regional Medical center for treatment of their injuries. Both occupants were wearing their seatbelts.

INCIDENT NARRATIVE: Yount was traveling Northbound on US95 near milepost 388.4 when he entered a curve in the roadway. As Yount was negotiating the curve he drove off the east shoulder, overcorrected and came back onto the roadway crossing both northbound and southbound lanes of US95 and drove off the west shoulder. The vehicle entered a ditch in a field on the west side of the road and rolled multiple times.
