On November 12, 2012, at approximately 6:00 p.m., the Idaho State Police investigated an injury crash westbound on Interstate 84 at milepost 46, at the top of the Eagle road offramp. Pamela Halvorson, age 65 of Meridian, was traveling westbound in a 2012 Kia Forte. Mark Oldham, age 53 of Orem Utah, was also traveling west in a 2012 Dodge van. Halvorson was taking the Eagle exit, and was stopped in traffic. Oldham was traveling at freeway speeds, became distracted, and rear ended Halvorson on the offramp. Halvorson's vehicle then proceeded to strike a fixed light pole. The collision caused the light pole to fall down across the westbound offramp and block traffic for about one hour.

Halvorson was transported by ground ambulance to St. Luke's on Eagle Road. The crash is under investigation by the Idaho State Police.
