On October 12, 2012 at approximately 2:40 a.m., Idaho State Police investigated a one-vehicle crash on US 91 at milepost 23, south of Swan Lake, in Bannock County, ID. Tyler J. Tew, 29, of Idaho Falls, ID., was driving north on US 91 in a 1997 Honda Accord. Tew collided with a cow in the travel lane of US 91, near Stockton Rd.

Tew was wearing a seatbelt. He was transported by ground ambulance to Portneuf Medical Center in Pocatello, ID. Alcohol is not believed to be involved in the crash.
