On October 11, 2011 at 5:01 p.m., Idaho State Police investigated a single vehicle injury crash westbound on SH33 at mile marker 58.7, near the Sage Junction Port of Entry.

Jeremy D. Duty, 32 of Riverton, WY, was traveling southbound on I15 in a 2005 Dodge pickup pulling a trailer. Duty had just exited at the 143 where he stopped at the stop sign but failed to yield to oncoming traffic on SH33. Kara L. Merkley, 21 of Jerome, was traveling westbound on SH33 in a 1996 Honda Civic, when Duty pulled out in front of her. Merkley turned the wheel in an attempt to avoid a collision and lost control of her vehicle. Merkley's vehicle left the roadway off the north side and struck a light post.

Merkley and her passenger, Sarah D. Dixon, 18 of Jerome, were transported by Air Idaho to Eastern Idaho Regional Medical Center for non-life threatening injuries. Both driver and passenger were wearing their seatbelts and both airbags were deployed. Duty was cited for failing to yield at a stop sign.

Corporal K. Barnes was assisted at the scene by Idaho State Police Sergeant Zaugg, Corporal Scoggins, Jefferson County Deputies, Roberts QRU, and Mud Lake Ambulance.
